Southern Tier AIDS Program, Inc. is seeking a Reentry Care Coordinator in our Binghamton office. This position provides confidential outreach, intake, and care management services and discharge planning to reentry participants. Assist with the general operations of the Reentry program; provides
program clerical and data management support.
This position is funded for only 1 (one) year and is not guaranteed to be funded after February 1, 2025.
Salary:
- $45,000/year, working 35-hours per week, may vary depending upon experience

- Associates degree in Health, Education, Social Work, Liberal Arts, a related field, and one years of qualifying experience; or Associate degree in Health, Education, Social Work, Liberal Arts, a related field, and two years of qualifying experience.
- Qualifying experience means verifiable full or part-time employment experience working with any of the following populations: persons with a history of mental illness, homelessness, and/or substance use, involvement and/or education with the criminal justice system.
- Must be very well organized, able to work in a fast paced environment, ability to prioritize without losing site of goals, attention to detail and accurate record-keeping, and communication skills are a necessity.
- Computer literacy is required.
This individual is responsible for meeting the transportation and insurability requirements of this job. A valid driver’s license, clean driving record and reliable transportation is a requirement of this position.
