POSITION: Recreation Coordinator/Case Manager

REPORTS TO: Director of Clinton Avenue Residence

CLASSIFICATION: Full Time

EXEMPT/NONEXEMPT: Salaried Non-Exempt

GENERAL DESCRIPTION:

The Recreation Coordinator/Case Manager seeks to improve and enhance the physical, mental, and emotional wellbeing of the residents through planning, developing, coordinating, implementing and supervising recreational activities. The Recreation Coordinator/Case Manager monitors and assesses the youths’ life skills levels and offers guidance and support. The Recreation Coordinator/Case Manager serves as a secondary/support Case Manager, providing case management services to the male juveniles detained by the Department of Juvenile Justice for non-secure detention within the Clinton Avenue Residence.

RECREATION COORDINATOR/CASE MANAGER MAJOR DUTIES AND RESPONSIBLITIES:

  • Acts so as to represent Lutheran Social Service of New York’s mission, values, ethics, and upholds agency Code of Conduct at all times.
  • Maintains regular contact with residents and families in accordance with OCFS and DYFJ mandates, including coordinating visitation and phone calls. Ensures that all contacts are documented via Progress Notes.
  • Plans, organizes, develops, schedules and evaluates activities for social, educational, recreational and physical development. Facilitates and supervises activities that not only encourage sportsmanship and positive behaviors but also those that teach conflict resolution and problem solving, development of interpersonal skills, behavior management, stress reduction, increasing self-esteem and self-awareness and achieving insight.
  • Develops programmatic activities at the facility, including holiday projects, summer programs, etc.
  • Arranges for community activities, including planning for safety, supervision, transportation needs and funding.
  • Maintains inventory of supplies and equipment, distributes supplies and equipment, assists with ordering supplies and equipment for recreation and athletic programs.
  • Conducts individual counseling and group discussions. Fosters a favorable group climate in which safety, emotional nurturing and healthy development are of primary concern. Maintains a non-judgmental and unbiased approach.
  • Maintains knowledge of the behavior program and how it applies to point system. Prepares corresponding reports for the youth record.
  • Conducts youth assessments and determines skills deficits. Provides skills training in areas including, but not limited to: daily living, self care and social relationships.
  • Obtains approval on all activities and volunteers from Director and/or as required by the agency or program funders.
  • Monitors court documents to ensure all court orders are fulfilled, including construction of adjustment reports.
  • Maintains resident file.
  • Refers residents for mental health and other support services and coordinates with staff to ensure compliance with appointments and services.
  • Attends DYFJ Case manager meetings, as scheduled.
  • Engages in crisis management and de-escalation with the youth and follows program Incident Reporting protocols.

QUALIFICATIONS

EDUCATION:

  • Bachelor’s Degree

EXPERIENCE:

  • Minimum of 3-years within the Human Service Field, preferably working in the area of childcare, security or correctional environment.

SKILLS & ABILITIES:

  • Demonstrates good oral and written expressive capabilities.
  • Displays Time Management skills, ability to handle multiple tasks and assignments at once, and flexibility, including working some holidays and weekends.
  • Must be enthusiastic, client-centered and able to work effectively with at-risk youth.
  • Presents capability of establishing therapeutic relationships with youth and positive working relationships within the Organization and with external agencies.
  • Able to maintain a safe and controlled environment.
  • Exhibits proficient knowledge of Internet Navigation, E-mail, and Microsoft Word.

OTHER REQUIREMENTS:

  • Possesses Crisis Intervention skills and engages in crisis management, as needed.
  • Maintains ability to establish appropriate boundaries with residents and other staff.
  • Shows willingness to participate in continuing education.
  • Valid NYS Driver’s License.
