• 2 years Neuro IRexperience 
• Experience with cerebral angiograms and thrombectomy within the last 2 years 
• Experience with scrubbing heart caths, pacemaker insertions, neuroand interventional radiology cases 
• COVID and Flu vaccination required -NO DECLINATIONS WILL Please add vaccination status in the submission notes
• BLS and ACLS at time of submission 
• 30 minute response time for call 
• Resume with all facilities listed
• Skills Checklist specific to the discipline (must be less than a year old)
• 2 supervisory references (completed within the last year)
• ActiveLicense (verification must beless than 30 days old)
• Certifications(must beactive at time of submission)
• Illinois license
• BLS (AHA)
• ACLS (AHA)
