POSITION SUMMARY:

Under the direction of the Director of Quality and Patient Safety, the Quality Improvement Specialistis responsible for the timely and accurate collection and analysis of data and information to support established clinical quality performance metrics, grants, operational goals, and regulatory compliance. The QI Specialist supports organization-wide efforts to ensure Quality Improvement initiatives are developed using a data-driven and evidence-based approach; provides guidance for Quality Improvement policy development; uses knowledge and application of various systems to generate quality reports and conduct regular opportunity analysis for key performance indicators; serves as a resource and coach for continuous Quality Improvement throughout the organization.

ESSENTIAL JOB RESPONSIBILITIES:

Quality Performance Improvement

  • Assists in developing and administering CFHC’s Quality Improvement Plan, monitoring progress and working with staff and leaders to achieve annual goals.
  • Leads performance improvement teams and workgroups using sanctioned performance improvement methodologies.
  • Uses analytic systems, databases and national benchmarks to continuously identify opportunities for improved outcomes.
  • Performs case review of patient care as defined by performance measures and standards of practice as needed. Assist with coordinating the professional peer review process as needed.
  • Utilizes current evidenced based practice to collaborate effectively with stakeholders in order to design operational and clinical practice processes in support of the organization's strategic quality goals.
  • In collaboration with the department Director, provides training and information sessions on Quality Improvement topics as needed.

Data Management

  • Maximize the utilization of all analytic systems and databases; effectively manage care gap reports in collaboration with Data Analysts.
  • Collects, compiles and analyzes data, serving as clinical quality measure subject matter expert (SME) by interpreting quality, patient safety, and service data.
  • Works closely with Data Analysts, designated data owners, and key stakeholders to ensure all quality data managed by the department is validated, accurate, and meaningful.
  • Assist with the administration of patient satisfaction surveys, results and dissemination of improvement plan as needed.
  • Coach different members of the care team and department staff to improve their clinical quality metrics and increase the number of providers and departments meeting quality initiatives and all quality goals.


MINIMUM REQUIREMENTS:

Education & Experience

  • A minimum of a bachelor’s degree required in Nursing, Business, or Healthcare related field.
  • A minimum of two (2) years’ experience in the healthcare industry.
  • A minimum of three (3) years’ experience in quality, patient safety and/or performance improvement.
  • Certification in Lean Six Sigma, CPHQ, or other quality certification strongly preferred.

Knowledge, Skills, and Abilities

  • Knowledge of policy formulation related to health care regulations.
  • Effective organizational, oral and written communication skills, problem solving, program development, computer skills, strong leadership, and team building skills.
  • Ability to work with a variety of disciplines and levels of staff across departments and the health system is required.
  • Strong critical thinking, collaboration, research, general business operations, and problem-solving acumen.
  • Deep knowledge of research, principles, and practices around diversity, inclusion, and racial equity.
  • Ability to meaningfully engage a wide variety of diverse stakeholders in challenging dialogue and critical self-reflection around racial identity, implicit bias, systemic racism, and structural inequities.
  • Passion and urgency for addressing racial inequities and improving the lives of underserved and underinsured populations within the surrounding neighborhoods of CFHC.
  • Ability to proactively identify challenges and barriers to stated objectives and employ assistance in addressing.
  • Solid working knowledge of computers including Microsoft Office Suite (Word, Excel, PowerPoint, and Outlook), Windows, and internet applications.
