Job description

The Quality Coordinator is a position for someone who is devoted and enthusiastic about quality processes and support, and who wants to join an organization and a dedicated Quality Team that is achieving results for clinical outcomes and life-changing service. This position will assist the Quality Team and Clinical Leaders at agencies across the country.
The Quality Coordinator helps the Quality Team and Clinical Support Team with their monthly, quarterly and yearly tasks as well as provide administrative duties for QRP requirements while working with vendors. They will also help Clinical and Quality Teams in meeting their goals The Quality Coordinator boosts and shares the responsibility of the department's work by showing co-ownership and accountability with all tasks related to Clinical and Quality department.
The Quality Coordinator role is part of the Pennant Quality Team and reports directly to the Director of Quality (DOQ). The Quality Coordinator will work with the Quality Team and other clinical support staff across the country. They will work together with those quality and clinical leaders to plan, track, and execute processes that improve the Quality and Clinical results.
DUTIES & RESPONSIBILITIES
Under the partnership of the Director of Quality and Quality Resources, the Quality Coordinator must adapt and gain understanding of multiple Pennant service lines, to include Home Health & Hospice and other service lines as needed. The ideal Quality Coordinator exemplifies ownership of all Quality functions and seeks to grow and develop Quality Functions for Pennant. Responsibilities and essential functions include:
Quality Reporting Program Functions
Ensure CAHPS compliance tracking, monitoring and storing. Assisting with HIS and OASIS QRP Compliance tracking, monitoring and storing. Ensure appropriate reporting and validation from Vendors to CMS 3rd party regulators for existing agencies. Partner with additional resources to support new start-ups, partnerships and acquisitions compliance with QRP. Assist in developing processes to ensure monthly follow-through of agency responsibilities around QRP. Conduct Regular Review of QRP Compliance
Data Support
Partner with department resources to support existing quality and clinical organization reports - includes monthly scorecards for clinical and quality outcomes. Partner with resources and agencies to develop and grow usability and adoption from field leaders and partners.
Other Quality Support Items
Work with Quality Partners to collect data to identify areas for improvement. Create and maintain comprehensive documentation, reports and records related to quality improvement activities. Assist in preparation of quality improvement meetings and training sessions. Support the implementation and monitoring of quality performance improvement projects. Assist in aggregate available data related to industry trends and best practices in quality improvement. Role may include some administrative functions to support Service Center Clinical Resources

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Education: Bachelors (Preferred)
Proficiency with Excel, competence with creating visibility and organizing data for stakeholder consumption.
Extensive experience in Microsoft Office Suite Applications -Outlook, Teams, Power Point, Word
Experience with Home Health and Hospice quality improvement concepts and practices preferred.
Ability to collaborate effectively with individuals at all levels of the organization.
Strong commitment to maintaining confidentiality and handling sensitive information appropriately.
Excellent interpersonal skills and the ability to work well in a team environment.
Strong problem-solving and critical-thinking skills and ability to adapt to changing environment.
Excellent communicator
High attention to detail. Responsible for many moving parts and projects.
Takes ownership in their results & processes.
Self-Driven. Doesn't rely heavily on permission or direction.
Professionalism in all aspects of interaction and communication at work
Compensation: Based on experience.
