JOB SUMMARY: Under the direction of the Performance Management Supervisor, the Performance Management Specialist assists with the implementation of the compliance, risk management, and performance improvement responsibilities of the Department of Human Services’ CUA contract standards and outcomes and Tabor Community Partner’s (TCP) agency’s performance improvement plan. This position requires collaborative work with leaders and staff at all levels of the organization, including the design and implementation of record audits on various standards, summarizing findings into performance improvement initiatives, and processing, reporting, and analyzing data.
EDUCATION: a bachelor’s degree in social services or a related field.
EXPERIENCE: A minimum of two (2) years of experience in program evaluation and monitoring of child, youth, and family serving systems. Preferred understanding of behavioral healthcare systems and organizations of care, regulations and standards, risk management, incident reporting, and performance improvement processes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Collects, aggregates, analyzes, interprets, and summarizes data in accordance with CUA’s quality improvement plan and cycle. Data collection activities include case file reviews, interviews, quality calls, and site visits.
- Conducts internal audits of case file reviews and interviews clients, service providers, and key stakeholders.
- Disputes and reconciles audit scores—internally and externally.
- Facilitates performance management-related trainings at New Hire Orientation.
- Reviews, tracks, and distributes reports received from DHS.
- Completes all data-related requests from DHS, PA DHS, other related funders, and the Performance Management Director and Supervisor that assist with improving the CUA’s quality and performance.
- Investigates the risk associated with staff-related service concerns and reports the findings to the performance management director and supervisor.
- Manages the incident reporting system to ensure timely submission and dissemination of reports to providers.
- Conducts monthly and quarterly analyses of incident reporting.
- Assists in developing plans of improvement for the agency.
- Conducts focus groups as assigned.
- Travels for trainings, meetings, and reviews, as needed.
- Assists with the collection and submission of documentation needed for Tabor Children’s Services licensure.
- Assists with all aspects of Council of Accreditation certification, as requested.
- Participates in meetings with DHS, PA DHS, and other stakeholders, as required.
- Participates in the required training.
- Participates in supervision.
