JOB DESCRIPTION AND POSITION REQUIREMENTS
Scheduled Weekly Hours: 40
Work Shift: (Monday thru Friday) 8 am to 5 pm
Summary of the Job:
Supports the Quality and Population Health Program by collecting and analyzing claims and quality data, encounters and other clinical data required to measure and improve program performance and appropriate code capture. Analyzes various quality data, provides input and knowledge based recommendations. Will have routine interactions with St. Tammany Quality Network (STQN) and St. Tammany Physicians Network (STPN) primary care providers, and other Quality and Population Health Program team members. Responsible for collecting, reviewing and analyzing data required to measure and ensure quality standards are met, and recommends and educates STQN primary care providers on actions for improvement to assist them to meet or exceed quality metrics and outcomes set by STQN and/or payers for incentive programs. Produces ad hoc reports using third party data resources in support of program's initiatives. Works closely with other decision support resources throughout STPN, STQN and other related entities such as health plans, third party data vendors, etc. Provides formal activities report and updates to the Quality & Population Health Manager on a monthly basis. Maintains HIPAA standards and ensures confidentiality of protected health information. Performs miscellaneous duties related to Quality and Population Health management as needed.
Minimum Qualifications:
Strong healthcare background with at least 3 years in hospital or ambulatory settings, clinical experience and/or CPC a plus. Strong medical economics analytical skills and familiarity with claims and clinical data and data systems.
Must have extreme attention to detail. Must have strong analytical, planning and organizational skills. Must be able to evaluate and establish priorities and objectives. Must demonstrate ability to accept and institute change. Must possess strong written and verbal communication skills. Must be capable of performing all position responsibilities without direct supervision. Must be able to quantify and educate ways to improve clinical performance in the areas of Quality and HCC outcomes. Must have teamwork skills in order to work with internal and external departments, as well as individuals. Must be able to prepare routine and ad hoc reports as needed. Capable of assuming new responsibilities and functioning independently within the scope of authority is required. Possess a process improvement focus that can be employed to continually enhance Quality and HCC outcomes. Must be able to work well with others and maintain a professional demeanor at all times.
Preferred Qualifications: Knowledge of and experience with coding terminology and risk adjusted methodologies preferred. Knowledge of and experience with EMR systems a plus, Epic experience preferred. Knowledge of and experience with HEDIS, CAHPS and Part D Safety measures preferred. Certified Professional Coder, preferred. Current LPN or RN nursing license, if applicable
Physical Demands:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required. Must be able to work with a moderate level of noise.
