As a Qualified Health Home Specialist on Nystrom & Associates' Behavioral Health Home (BHH) team, you will be part of a community based team providing case management services for individuals with a mental health diagnosis. You'll make a major impact on your local community by helping individuals get connected to mental health services, preventative care, and community programs.
Behavioral Health Home (BHH) is a lower level of care and considered a step-down from Adult Rehabilitative Mental Health Services (ARMHS), Targeted Case Management, or Assertive Community Treatment (ACT). BHH is a team-based model that includes a case manager, nurse and a community health worker.
Payrate: $18 - $23 per hour (DOE)
The Qualified Health Home Specialist, Nurse & Case Manager work as a team, and will work with clients on goals related to:
- Connection to mental health services, preventative care, and community programs such as housing, food shelves, employment, etc.
- Transitions in care
As a Qualified Health Home Specialist, you'll have the opportunity to:
- Provide culturally responsive, holistic care in the community.
- Work closely on a team with nurses and case managers.
- Provide clients with health promotion services by assisting them with skills related to overall health and wellness
- Interact with clients on the phone or in-person monthly.
Requirements
Required Qualifications:
If BHH services are offered in either a primary care setting or mental health setting, the Qualified Health Home Specialist must meet one of the following qualifications:
- Completion of the Community Health Worker certificate program through an approved college or technical school in Minnesota
- A mental health certified peer specialist who is qualified according to Minnesota Statutes, 245I.04, subdivision 10 Behavioral Health Home (BHH) Services Standards 3 3. Culture to support integration
- A mental health certified family peer specialist who is qualified according to Minnesota Statutes, 245I.04, subdivision 12
- A case management associate as defined in Minnesota Statutes, 245.462, subdivision 4, paragraph (g) or Minnesota Statutes, 245.4871, subdivision 4, paragraph (j)
- A mental health rehabilitation worker who is qualified according to Minnesota Statutes, 245I.04,subdivision 14
- A community paramedic as defined in Minnesota Statutes, 144E.28, subdivision 9
- A peer recovery specialist as defined in Minnesota Statutes, 245G.07, subdivision 1, paragraph (a), clause (5)
- A community health worker as defined in Minnesota Statutes, 256B.0625, subdivision 49
- Valid Driver's License, Auto Insurance, and Reliable Vehicle; access to the internet
- Ability to work independently; ability to drive a minimum of a 30-mile radius around primary clinic location
Nice to have:
- Previous experience working with diverse communities
- Interest in working with clients who have a mental health diagnosis
- Knowledgeable of community resources and health interventions
- Desire to work with children and families and/or adults in their homes; this includes walking, stairs, and the ability to crouch, stoop, sit on floors, and stand for periods of time
- Must be proficient in the use of computers, including Microsoft Word, Excel, Outlook, and Electronic Health Records (EHR)
