Conduct one on one sessions with our clients and connect them to the services in their community to thrive.
Responsibilities:
- Provides intensive Crisis Services in a one-to-one setting for individuals experiencing a crisis situation with emphasis on linking the individual to services and coordinating care.
- Implement crisis interventions and service plans.
- Models and teaches problem-solving techniques
- Coordinates and provides transportation to services and medical appointments as needed.
- Coordinates scheduling of appointments with the individual and family.
- Provides 24-hour crisis intervention and support for the individual and family as necessary.
- Documents all service delivery in a timely manner as required by the Department of Behavioral Health and Developmental Services.
- Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in delivering services to the individual and his/her family.
- Actively coordinates and plans discharge and aftercare plans.
- Attends interagency meetings and supervision sessions as necessary.
- Attends and participates in orientation, in-service training, and conferences as scheduled.
- Performs other duties as assigned.
Knowledge, Skills and Abilities:
- Familiar with assessments tools
- Knowledge to develop and implement service plans.
- Knowledge of counseling and crisis intervention strategies.
- Effective written and verbal communication skills
- Strong interpersonal, communication, and documentation skills
- Behavior management training and the ability to manage challenging behaviors and implement positive behavior support plans.
- Knowledgeable of community-based services and supports.
- Effective delivery and presentation methods and styles.
- Strong time management and organization skills.
Qualifications:
- Must be able to function as a Qualified Mental Health Professional-A as determined by the state of Virginia regulations
- Excellent written and verbal communication skills are required
- CPR and First Aid certified
- Behavior management skills
- Knowledge of psychotropic medications
- Must submit to and receive satisfactory results from a criminal background investigation and a registry check by the Department of Social Services
- A valid driver’s license and current copy of DMV driving record
- A copy of the declaration page of automobile insurance is required
