QUALIFICATIONS:


Graduate of an accredited nursing school. Must possess a current license to practice nursing in the state of practice. Minimum of one (1) year of experience in general nursing practice, hospice experience and/or palliative, end of life care. Should have supervisory and good communication skills. Ability to work well with others, function well under pressure, and function independently. Must comply with the standards outlined in the Nurse Practice Act.


Bachelor degree preferred as well as Coding experience. Thorough knowledge of current federal and state regulations. three years preferred. Two (2) years previous experience with Quality Improvement required. Must have the ability to multi-task and complete all assignments on a timely basis by using good organizational skills and management of priorities. Must be proficient in hospice patient multi-disciplinary case management. Must have excellent documentation skills and have the ability to review documentation identifying compliance and case management.

REPORT TO: Director of Patient Care Services

 

KNOWLEDGEANDCOMPETENCY:


Must demonstrate basic knowledge of all procedures included in the Registered Nurse description. Must have the ability to follow instructions from the physician, nursing supervisor and other professional staff. Ability to communicate effectively with managers, patients, families and co­ workers. Must be able to assess safety of home situations for self, patients, and other staff members, including physical and psychological dangers. Must have the ability to effectively cope with patients, families and all others with varying backgrounds, socioeconomic conditions, and value judgments.


Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient's care.


JOB RESPONSIBILITIES (includes but not limited to the following):

  1. Performs the quality improvement activities for hospice as outlined in the QAPI plan, working closely under supervision of the Director of Patient Care Services.
  2. Assures that desired patient outcomes are achieved and sustained, by such mechanisms as chart review and staff education.
  3. Completes ongoing monthly requirements of QAPI plan, including performing data collection on all chart reviews related to adverse event outcomes.
    1. Compiles data, analyzes trends, and rep01is minimum of monthly to Director of Patient Care Services.
  1. Assures that all patient records exhibit evidence of quality multi-dissiliency case.
  2. Assures through quality improvement activities compliance and standards of care for federal, state and local regulations is
  3. Educates staff regarding performance improvement and involves staff in performance improvement activities.
  4. Works closely with Medical Records Coordinator to implement activities designed to improve documentation, including assurance that process for tracking of physician orders is followed and orders are signed and in records on a timely basis.
  5. Works closely with Director of Patient Care Services to develop and implement further activities designed to improve patient outcomes.
  6. Performs chart audits on a timely and concurrent basis to ensure that all documentation is in place, content is appropriate for quality case management, regulat01y requirements are.
    1. Written results of chart audits are shared consonantly with Director of Patient Care Services
    2. Staff is educated to expectations of appropriate.
  7. Perf01ms field homecare visits in addition to QI activities, as scheduled by Director of Patient Care Services, to include:
    1. Initial opening assessments on patients
    2. Supervisory visits
    3. Other patient visits as scheduled by Director of Patient Care

PHYSICALDEMANDS:


Must be able to complete all physical demands of the job, which may include, but not be limited to: the ability to lift and transfer patients, carry supplies, and read n01mal typewritten print. Have connected vision and hearing within normal range and have manual dexterity with normal range of motion of all extremities. May be requested to. Work weekends, holidays, and occasional ove1iime. Must be able to work alone without usual suppmi systems immediately available in acute care settings. Must have the ability to deal with abrupt schedule changes and to maintain objectivity in coping with the stress of working with acute, chronic and terminally ill patients.
