POSITION SUMMARY
The Public Health Nurse (PHN) is part of a multidisciplinary team working together to determine, assess, establish, and carry out a Public Health and case management plan for each client, with an emphasis on health education and health services that are preventative in nature. The PHN will advocate for positive health changes in the UAII patient community of the Los Angeles metropolitan region. The Public Health Nurse will have the opportunity to work directly with patients to improve their health outcomes.
ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES
● Identify and respond appropriately to the health care needs of all patients who are onsite at UAII, includes both scheduled and walk-in appointments.
● Provide health screening of patients of all ages and refer to clinicians for diagnosis and treatment as indicated and per scope of RN practice
● Assist patients in accessing primary and specialty care in partnership with case managers and community health workers.
● Maintain an active list of all patients sorted by geographic area and disease registry. Follow up and conduct in-reach to patients as indicated.
● Oversee case management and Public Health outreach efforts to maintain appropriate treatment plans and continuity of care.
● Provide health education for the purpose of primary, secondary, and tertiary care prevention
● Responsible for PHN encounters, face-to-face evaluations, education & screening, and home visits as indicated
● Responsible for percentage of chart reviews for quality assurance purposes
● Maintain accurate and timely documentation of all encounters. Complete documentation within 24 hours of patient visit.
● Collaborate with Care Team members such as the Health Educator, Dietitian, Dental, Behavioral Health, etc.
● Adhere to performance indicators related to the delivery of services, and nursing services
● Submit required reports as requested by internal and external sources
● Monitor local epidemiology trends as indicated by local and federal agencies to adapt preventative measures for community health as needed
● Responsible for supporting and/or providing active feedback on program goals and plan enhancements to improve community health outcomes
● Keep Director well informed regarding activities, pending issues and potential problems
● Participate in review and updating of Medical Protocols, Policies, and Procedures as needed
● Actively participate in Site Visits, audits, grant proposals and reporting, as requested
● Attend required departmental and agency meetings as requested
● Other duties as assigned or required to fulfill the purpose of the position
MINIMUM QUALIFICATIONS Qualifications:
● Experience with Electronic Health Record system
● Experience with immunization administration and documentation
Education &
Experience:
● Bachelors’ degree required, Bachelor of Science Degree in Nursing (BSN) preferred
● Minimum of 1 year in direct patient care
Certifications:
● Current unrestricted California Registered Nursing License
● Current California Board of Registered Nursing, Public Health Certificate
Confidentiality:
● Maintain strict confidentiality of client information adhering to HIPAA, professional codes of conduct, State of CA regulations, Privacy Act, and UAII policies and procedures
● This position has access to sensitive information, and a breach in confidentiality will be grounds for disciplinary action up to and including immediate termination
Communication/ Interpersonal Skills:
● Communicate and deal effectively with individuals in a professional and courteous manner verbally and in writing
● Proficient in English
● Demonstrable ability to lead workshops in a public forum, including various age groups
● Provide care in a culturally competent and sensitive manner for the American Indian/Alaska Native population
Technology Skills:
● Working knowledge of Microsoft Office Suite (Word, Excel, PowerPoint)
● Working knowledge of virtual video platforms (Zoom, Google Meets)
● Demonstrable ability to learn RPMS databases and associated applications as needed
Other:
● Available and willing to travel as needed for training and presentations
● Available to work occasional evening and weekend hours as needed
● Willingness to learn new skills and participate in training sessions relevant to the position, both internal and external
● Maintains personal responsibility for maintaining a valid RN licensure and for ensuring timely completion of appropriate Continuing Education Units for renewal of license
POSITION PREFERENCES
● Experience working with the American Indian/Alaskan Native community
● Experience working with nonprofit organizations, community groups, and performance-based government contracts
● Knowledge of Indian Self-Determination and Education Assistance Act (Public Law 93-638)
● Minimum 2 years’ experience in Public Health field
● Minimum 2 years’ experience in a community clinic setting

There are prolonged periods of sitting, keyboarding, reading, and driving or riding in transport vehicles. The employee must occasionally lift and move up to 20 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with staff and vendors.
