Public Health Nurse-LPN
Job Summary:
The Public Health Nurse specializing in immunization outreach and education at the City of Danbury Department of Health and Human Services is a dedicated professional responsible for spearheading immunization outreach initiatives, administering vaccines, and educating the local community on the importance of immunizations. This role plays a critical part in enhancing immunization rates and safeguarding Public Health within Danbury. You will work under the direct supervision of the Director of Health and Human Services and the Associate Director of Community Health, or their designee.
Key Responsibilities:

• Identify and target populations within Danbury with suboptimal immunization rates.
• Organize and lead outreach programs, events, and campaigns to promote vaccination.
• Collaborate with community organizations, schools, and healthcare providers to reach underserved populations.
• Establish and maintain relationships with local vaccination clinics and healthcare partners.
• Provide evidence-based information and guidance to individuals and families about immunizations.
• Address vaccine hesitancy and debunk misconceptions related to vaccines.
• Offer detailed information on vaccine schedules, potential side effects, and vaccine safety.
• Adapt and customize educational materials to cater to the diverse cultural and linguistic needs of the community.
• Administer vaccines to individuals in compliance with established immunization protocols.
• Ensure safe and accurate vaccine administration practices.
• Maintain accurate vaccination records and monitor follow-up vaccinations as required.
• Collaborate closely with healthcare professionals to ensure the appropriate and timely delivery of vaccines.
• Maintain meticulous records of vaccine administrations and educational outreach activities.
• Continuously monitor and evaluate immunization coverage rates within the Danbury community.
• Regularly report collected data to health authorities and department supervisors.
• Utilize data-driven insights to design targeted immunization outreach strategies.

Qualifications:

• Bachelor's degree in Nursing from an accredited program.
• Current Licensed Practical Nurse (LPN) licensure or in the state of Connecticut.
• Public health nursing certification or relevant training is highly desirable.
• Previous experience in Public Health, community health, or immunization outreach is preferred.
• Profound knowledge of immunization schedules, vaccine-preventable diseases, and vaccine safety.
• Exceptional communication and interpersonal skills.
• Cultural competence and a demonstrated ability to engage effectively with diverse populations.
• Proficiency in basic computer applications.
• Bilingual proficiency in Spanish and English is highly desired.

Additional Requirements:

• Possession of a valid Driver's License and access to a personal vehicle for local travel.
• Willingness to work flexible hours, including evenings and weekends, as necessary to support outreach activities.

Terms of Employment:
This position falls under the "Special Fund" category and is non-tested. Employment is contingent upon the availability of future funding.
