POSITION TITLE: Public Health Nurse
DEPARTMENT: Health Department
REPORTS TO: Director of Health
CLASSIFICATION: Hourly; Unaffiliated; Budgeted at 8 hours per week, with additional hours available based upon grant availability and funding
JOB SUMMARY:
The primary responsibility is to manage and implement the health promotion and disease prevention programs for the Town of Monroe.
ESSENTIAL DUTIES & RESPONSIBILITIES:
· Assists the Director of Health in organizing, scheduling, coordinating and implementing department Public Health programs
· Provides educational outreach and resources on various Public Health topics for the community through presentations, social media, and information booths as required.
· Conducts investigations and follow-up of communicable/reportable disease cases. Ensures proper referrals for the treatment of individuals to reduce the potential spread of disease into the community
· Coordinates and implements immunization program and provides immunizations as required
· Maintains records of reportable diseases and investigations, as required by state statutes and/or regulations
· Manages inventory of medical supplies and vaccines
· May assist in coordination of efforts among other staff, interns, volunteers and/or Town officials in the implementation of Public Health programs
· Maintains close communication with Monroe Public Schools nurses for the prevention and response to outbreaks of illness
· Works closely with other staff during the investigation of outbreaks of food borne illness and other diseases which may be caused by environmental factors
· Prepares reports and significant findings for the Director of Health, Connecticut Department of Public Health, and others as appropriate
· Works closely with the Director of Health for the management of grant deliverables and financial reporting for grant-supported programs
· Works with town employees, volunteers, and stakeholders on various special projects related to community and/or environmental health
· Available to respond to Public Health emergencies and participates in emergency preparedness training, drills, and exercises.
· Required to travel in the course of daily work and must possess a valid Connecticut driver’s license
· Required to keep abreast on current Public Health standards and practices
· Performs other duties as assigned
EDUCATION/QUALIFICATIONS:
· Three years of experience as a licensed Registered Nurse, including one year of experience as a Public Health nurse/educator
· Must have a current, unencumbered Connecticut license as a Registered Nurse
· Four-year college degree in, Nursing, Public Health, Health Education or related field
· Considerable knowledge of the principles, practices and techniques applied infectious disease control and Public Health education
· Ability to prepare written reports containing findings, analysis, conclusions and corrective recommendations
· Experience and/or certifications in Public Health emergency preparedness planning and response is a plus

Pay: $45.00 - $55.00 per hour
