Job description

Reporting to the Public Health Nursing Supervisor, the Public Health Nurse provides population-focused, community-oriented nursing care. Public health nursing practice is based upon the nursing process, the tenets of Public Health nursing, and the core functions of Public Health, assessment, policy development and assurance.
The mission of Outagamie County Public Health is to use local data to prevent disease and injury, promote wellness, and protect the health of our communities through collaboration and best practice.
Core business hours are Monday through Friday, 8:00AM to 4:30PM. Work schedule requires flexibility to work outside of core business hours which include evenings and weekends.

The following duties are most critical for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

  • Performs surveillance, prevention, investigation, and control of communicable, acute and chronic disease.
  • Enforce State and Federal laws and regulations that protect health and ensure safety.
  • Identifies population groups, families, and individuals at high risk of illness, injury, disability or premature death.
  • Collaborates with public and private organizations to address community health needs.
  • Collects, reviews and analyzes health status data.
  • Assesses community strengths and needs.
  • Develops policies and programs to address Public Health priorities.
  • Provides case management and nursing care to individuals and families utilizing the nursing process.
  • Provides community screening, health promotion and immunization services in cooperation with the public and private sector.
  • Provides Information and Referral services to the general public and medical and social service community.
  • Evaluates the effectiveness, accessibility and quality of health care services.
  • Informs, educates and empowers people about health issues.
  • Maintains confidentiality and security of protected health information.
  • Integrates quality improvement into daily activities and represent Public Health Division by participating in coalitions and professional meetings. Promote professional growth to increase personal knowledge base to better serve the agency and community.
  • Complete and maintain applicable training/certificates to support emergency response roles and responsibilities. Maintains emergency preparedness core Public Health competencies by participating in preparedness training exercises and planning events.
  • Participates in Divisional Quality Improvement, Performance Management, Strategic Planning, Workforce Development, Community Assessment, Community Health Improvement Planning activities
  • Provides practicum experiences for nursing students and resident physicians.
  • Participates in cross-training to perform intervention strategies aimed at creating healthy environmental, social, and economic conditions to benefit the health of the population.
  • Maintains regular and predictable attendance.
  • Evening and/or weekend work and/or extended hours and irregular shifts may be required.
  • Provides on-call services as assigned.
  • Performs other duties as assigned.
  • Bachelor of Science degree in Nursing (BSN) from an accredited nursing program and one to two years clinical and Public Health nursing experience.
  • Valid American Heart Association Basic Life Support (BLS) certification
  • Requires a valid WI driver's license, vehicle to provide transportation to and from job assignments, appropriate vehicle coverage and maintain driver's eligibility of employment.
  • Knowledge of the Core Competencies for Public Health Professionals. (http://www.phf.org/resourcestools/Pages/Core_Public_Health_Competencies.aspx). Developed by the Council on Linkages, the Core Competencies reflect skills that are desirable for professionals who deliver the Essential Public Health Services. The Core Competencies serve as the foundation for Public Health practices and are intertwined in the key responsibilities listed above.
  • Knowledge of and maintenance of Public Health Preparedness and Response Core Competencies by participating in preparedness training, exercises, planning events and responding to actual events.
  • Ability to operate a variety of medical equipment including anemia, cholesterol, and blood sugar screening equipment, stethoscope, thermometer, scale, needles and syringes, etc.
  • Ability to use and interpret medical terminology.
  • Ability to comprehend and interpret a variety of documents including statistical reports, referrals, client records, lab reports, policy manuals, medical references, etc.
  • Ability to assign, supervise, and review the work of unlicensed personnel.
  • Ability to add, subtract, multiply, divide, calculate decimals and percentages, rates, ratios, and make use of the principles of high school algebra.
  • Ability to communicate effectively with health care providers, community organizations, school personnel, governmental officials, patients and families, and others verbally and in writing.
  • Ability to mobilize interdisciplinary partnerships.
  • Ability to prepare performance objectives and conduct program evaluation.
  • Ability to lift, carry, push, pull and move objects up to 50 pounds, understanding and utilizing proper body mechanics.
  • Ability to maintain accurate and timely records.
  • Ability to prepare a variety of documents including letters, referrals, evaluation reports, etc.
  • Ability to transport self to and from various locations on demand.
  • Operate business computers and office machines including Microsoft Office products such as Word, Excel, and PowerPoint.
  • Ability to work independently.
  • Bilingual capability preferred.
  • Work is generally performed in a medical office, home setting or office setting with exposure to potentially dangerous materials and situations or blood borne pathogens that require following extensive safety precautions and may include the use of protective equipment.
  • Exposure to inclement weather may occur.
  • Exposure to Latex may occur.
