Public Community Health Nurse

Hourly Wage: $46.10/Regular/Full-Time
A Public/Community Nurse is a highly skilled healthcare professional who plays a vital role in improving the health and well-being of individuals, families, and communities. This role involves providing a wide range of healthcare services in community settings, addressing the unique healthcare needs of diverse populations. Public/Community Nurses focus on health promotion, disease prevention, and delivering patient-centered care to support the overall health of their communities.
Examples of Work Performed:
Conduct comprehensive health assessments of individuals and communities to identify healthcare needs and risks.
Develop individualized care plans based on assessment findings, addressing physical, emotional, and social aspects of health.
Educate individuals and groups on preventive measures, healthy lifestyle choices, and disease management.
Organize and participate in Community Health promotion activities, workshops, and events.
Administer immunizations and vaccinations to prevent the spread of communicable diseases.
Conduct screenings for various health conditions, such as hypertension, diabetes, and sexually transmitted infections.
Provide skilled nursing care, wound care, medication management, and monitoring of chronic conditions for homebound individuals.
Collaborate with healthcare teams to ensure coordinated care for patients with complex medical needs.
Establish strong relationships with community members, leaders, and organizations to identify and address healthcare disparities.
Advocate for Community Health needs and participate in local health committees and initiatives.
Play a crucial role in disaster preparedness and response, including providing immediate care and support during emergencies.
Maintain accurate and up-to-date patient records and Community Health data in IHS Electronic Health Records, which are maintained by IHS.
Compile and submit reports on public health trends, outcomes, and interventions.
Respect and address cultural, social, and economic factors that influence health disparities within the community.
Stay current with best practices, healthcare trends, and regulatory changes through ongoing professional development.
Participate in quality improvement initiatives to enhance the delivery of healthcare services within the community.
Provides in-home wound care and assessments.
Receive, coordinates, and provide any updated health information, new medication, and new physician orders from outside entities, and IHS to appropriate health professionals.
Assist patients and programs in coordinating with other Tribal Health and Home Health programs, and local community programs.
Aide in monthly Nail Care clinics that the former Home Health provides.
Other duties as assigned.
Knowledge, Skills and Abilities:
Knowledge of and the ability to apply nursing principles, practices, and procedures required to assess patient needs in all aspects of health as well as knowledge of pathophysiology, anticipated complications and indicated therapeutic interventions.
Relevant experience in community or public health nursing.
Strong interpersonal and communication skills.
Knowledge of public health principles, epidemiology, and healthcare policies.
Cultural competence and the ability to work with diverse populations.
Proficiency in electronic health records and healthcare software.
Knowledge and skill in operation of specialized medical equipment.
Ability to work independently in both clinical and public health nursing practice. This includes the ability to work on own initiative with minimal supervision, prioritizing and completing projects, assignments, and work duties in a timely manner and being innovative in creating new systems for optimal patient care.
Ability to receive medication orders and changes accurately from health care providers.
Minimum Requirements:
Registered Nurse (RN) licensure and Bachelor of Science in Nursing (BSN) required.
Current Active RN, BSN State of Washington license with no record of disciplinary actions.
Certification in public health nursing or related field is an asset.
Up to date on CEU's per state requirements.
Up to date on first aid/CPR certification.
Must pass Indian Health Services background check.
Must pass a pre-employment background check.
Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.
Required to pass a pre-employment drug test.
