If this sounds like a good fit to you, we are seeking applicants for a Case Aide - Psychosocial Rehabilitation Specialist (PSR) to join our Coeur d'Alene field office on our Child Welfare team.
General Information:
+ The PSR Case Aide position requires a bachelor's degree in social work, behavioral science, education, or healthcare from a nationally accredited university or college.
+ The PSR Case Aide must be able to work autonomously and within teams while gaining extensive knowledge of Child and Family Services activities, programs, and policies.
+ The PSR Case Aide must display reliability, excellent communication skills, attention to detail, effective business writing skills, a high level of professionalism, and an exceptional ability to successfully coordinate and complete multiple tasks.
+ Punctuality and dependability are key for this position.
The Idaho Department of Health & Welfare is a drug-free workplace. ? EXAMPLE OF DUTIES:Child Well-Being Support:
+ Tracks child medical appointments and coordinates follow-up services for vision, dental, medical, and medication care.
+ Collects medical records and uploads to electronic file system.
+ Completes, submits, and tracks residential admissions paperwork, to include Medicaid process.
+ Makes referrals to Idaho Infant Toddler Program, per Standard guidance.
+ Completes formal referrals for mental health services.
+ Assists with coordination of school transportation for foster youth.
+ Administers Independent Living assessment tool and make Independent Living referrals.
+ Coordinates parent participation in child routines and activities.
Concurrent Planning Support:
+ Organizes family team meetings and coordinates follow-up tasks.
+ Completes Child and Family Social and Medical Information Forms, to include paternity identification and assists with process establishment; Tribal identification and assists with sending
