Job Description
- Maintain a client caseload and productivity by maintaining regular contact with clients to monitor and assess progress
- Identify client’s strengths, needs, and abilities and address barriers to meeting client goals
- Use a strengths-based and trauma-informed approach to work collaboratively with clients and internal and external team members to help support clients in their treatment and recovery
- Assist clients with community integration, transportation, and life skill development
- Goal setting and treatment planning
- Linkage with community resources and referrals
- Provide supportive services that encourage independence
- Medication monitoring and crisis intervention
- Document all activities in a timely and accurate manner
- Remain in compliance with internal and external policies, procedures, regulations, and standards, including all of the agencies policies and procedures and Quality Assurance Plan, and State Licensing standards
- Participate in all assigned meetings, staff development, and training as required
- Associate or Bachelor’s degree in Social Work, Human Services, or related field with 1 or more year(s) experience in an AOD or mental health setting
- Case management experience and working knowledge of community resources
- Strong interpersonal and engagement skills
- Strong organizational and time-management skills
- Excellent written and verbal communication skills
- Strong computer skills required; experience with documentation in EMR
- Ability to support the agency’s mission and philosophy, and demonstrate sensitivity to cultural diversity and workplace harmony
