Johns Hopkins Hospital is hiring for a Psychiatric Social Workerfor the Adult Psychiatry Department.
Schedule: Monday-Friday 8am-5pm
Requirements
- Employee demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her unit as indicated by the following:
- Demonstrates ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs.
- Identifies the physical, motor, sensory, cognitive and psychosocial characteristics.
- Demonstrates age appropriate communication skills.
- Demonstrates ability to provide the care needed as described in the job description and the department's scope of service for the patient groups under his/her care.
- Age group(s) served will be one or more of the following: Infant (Birth to 12 months), Child (13 months to 12 years), Adolescent (13 years to 17 years), Adult (18 years to 69 years) or Geriatric (70 years and older).
- Collaborates with Clinical Manager to develop therapeutic program for partial hospitalization.
- Conducts group, individual and family therapy sessions for the partial hospitalization program and other psychiatric settings as needed.
- Performs approved liaison, teaching, and outreach activities, and committee assignments which underscore professional practice and enhance the goals/objectives of the Hospital and department.
- Represents the Hospital in collaboration with patients, family, the Care Team and
- Interdisciplinary Care Team to assure appropriate, safe, cost-effective, timely and efficient care.
- Identifies patient and family needs and arranges for consultation with specialty services needed, including, but not limited to, medical, financial, environmental, procurement of services and discharge planning.
- Serves as an advocate on behalf of patients and families. Communicates with patients to ensure understanding of third party payer guidelines.
- Performs patient intake screenings and document per Hospital policy.
- Acts as a liaison to external care providers and community health resources, participates in patient care conferences, assures completion of discharge-related functions on a timely basis and assures patient and family education needs are met.
- Communicates and negotiates with appropriate third party payers to obtain necessary pre certification.
- Provides leadership and guidance to the care team in individualizing and implementing the plan of care.
- Clinically evaluates psychiatric patients admitted through Emergency Department. Assist Emergency Department physicians and staff in maximizing efficiency in assessment and dispositional planning. Coordinate planning with 3rd party payers and treating/on call psychiatrist.
- Provides crisis intervention as needed.
- Provides ongoing assessment, clinical intervention and discharge planning coordination for patients admitted to psychiatric unit and partial hospitalization program.
- Performs utilization management function in absence of Case Manager for psychiatry.
- Provides consultation on the medical service for physician and staff regarding patients with psychiatric issues.
Qualifications- MS Social Work from an accredited school of social work required.
- Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LCSW-C level.
- Minimum of 2 years of experience as a Social Worker in a hospital setting.
- Experience with inpatient therapy, outpatient therapy, and discharge planning is required.
- Current BLS CPR certification is required.
