Program Overview
Henry Street’s newest Health & Wellness program, CONNECT (Continuous Engagement between Community and Clinic Treatment), serves our most vulnerable adult communities with mental health, housing, and employment support in a fluid model that meets clients where they are. This project aims to provide rapid access to services and a more flexible and holistic approach to mental health that is more engaged with the community and the whole person outside the clinic walls and beyond the traditional doctor/therapist-patient interaction. The purpose of CONNECT is to improve the quality of life of the people served as demonstrated by progress towards recovery goals and informed by process metrics related to improved access, engagement and retention in clinical and non-clinical services and reduced mental health emergency, inpatient, and mobile crisis utilization.
Job Summary:
As a Psychiatric Nurse Practitioner in our clinic, you will provide clinical services to individuals, groups, and families. You will be asked to work in an interdisciplinary team and be assisted by two nurses. This is a community mental health clinic with the mission to be accessible to the Lower East Side community. This position requires some work in the field providing medication management outside the clinic for a few selected clients.
Qualifications:
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) required.
- New York State NP License with specialty in Psychiatry
- Experience with Mobile Treatment (ACT, IMT or Mobile Crisis team) preferred.
- Must be interested in Community Mental Health
- Background in Addiction medicine or willingness to obtain MAT (Medication Assisted Treatment) training.
- Must be able to practice independently.
- Excellent knowledge of psychotropic medications
- Excellent clinical knowledge
- Excellent organizational and verbal and written communication skills
- Bilingual preferred- Chinese (Mandarin) or Spanish/English
- Computer knowledge and efficiency essential, willing to use EHR.
- Must be a team player.
Responsibilities:
- Perform Psychiatric Assessments for new patients in Article 31/ MHORTS Outpatient clinic.
- Attend Intake review meetings, incidents review meetings and QA meetings.
- Be part of the CONNECT team and do some work in the field.
- Oversight of assigned clinic patients and their Psychotropic Medications.
- Evaluate patients for psychotropic medication.
- Evaluate crises for hospitalization.
- Prepare referral letter to designated hospital if hospitalization is required.
- Meet with Director and co-director about treatment issues and treatment protocols.
- Review and sign Treatment plans
- Provide MAT treatment to a few selected patients.
- Endorse a holistic approach to the recovery of patients.
- Other related duties as assigned.
Essential Physical Job Functions:
- Ability to walk on sidewalks, up and down stairs, and throughout clinic consistently throughout each day
- Ability to carry folders, a tablet, a notebook, and cell phone (up to 20 pounds) for several hours per day
- Ability to work onsite in the Lower East Side and travel in the area as needed
- Fully vaccinated against COVID-19
