Bluestone Physician Services is an innovative primary care practice specializing in the care of patients within assisted living and memory care communities. We are seeking an experienced Nurse Practitioner with the specialty of psychiatry/Mental Health who enjoys working with individuals with chronic and persistent mental illnesses and dementia.
We invite you to join our Psychiatric Nurse Practitioner team where you will partner with our Primary Care providers and Clinical Social Workers to bring psychiatric and behavioral health support to our patients that reside in assisted living and memory care facilities in north metro area of Milwaukee, WI
What you can expect:
- Provide on-site psychiatric/behavioral and telehealth care to patients in group homes, assisted living, and memory care facilities
- Full-time providers care for patients Monday-Friday; No Call
- Manage approximately 55-60 visits per week
- Develop and implement care plans appropriate to patients and their goals
- Clinical support from clinical assistants and care coordination through LPNs, RNs, and social workers
- Collaborate and Partner with Bluestone's primary care providers and Behavioral Health Integration Team as well as community partners
Qualifications:
Minimum Qualifications
- Graduate of an accredited Nurse Practitioner Education Program
- Board Certified/Board Prepared through ANCC
- Licensed or ability to obtain licensure within the state of Wisconsin to practice
- Ability to obtain and /or maintain DEA certification for the State of Wisconsin
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
- Must have a valid driver's license and minimum state required liability auto insurance; local travel is required
Preferred Qualifications
- 2+ years at a psych/Mental Health Nurse Practitioner
- Board Certified by the ANCC in Psychiatry Mental Health
