Cordial Health is in search of a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to become a valuable part of our team. **Key Responsibilities:**
1. Perform thorough psychiatric evaluations and assessments of elderly patients within nursing homes.
2. Formulate diagnoses and create personalized treatment plans tailored to each patient's Mental Health requirements.
3. Administer medication management and make necessary adjustments to treatment plans.
4. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure a holistic approach to patient care.
5. Provide counseling and psychotherapy services to patients and their families.
6. Educate patients and their families about Mental Health conditions, treatment choices, and self-care strategies.
7. Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
8. Participate in clinical meetings, case conferences, and quality improvement initiatives.
**Qualifications:**
1. Possession of a valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
2. Ability to work independently and efficiently manage a caseload of patients.
3. Strong diagnostic skills and a thorough understanding of psychiatric medications and treatment modalities.
4. Excellent interpersonal and communication skills for building rapport with patients, families, and members of the interdisciplinary team.
5. Proficiency in electronic health record (EHR) systems and basic computer skills.
6. A compassionate, patient-centered approach to care.
