Permanent Supportive Housing Case Manager

Job Description

Reporting to the Director of Case Management, the Permanent Supportive Housing Case Manager provides trauma-informed, intensive case management services to participants in the Permanent Supportive Housing (PSH) program. Services include conducting intakes and assessments, securing housing, providing referrals for supportive services within the community, implementing crisis management strategies, and helping all participants to maximize self-sufficiency. This position is a full-time position under the PSH grant.

Case Manager Skill Set

• Assesses, performs intakes, and works with people to address barriers to housing, creating strength-based individualized case plans to achieve goals and providing follow up services.
• Implements evidence-based, trauma-informed case management practices.
• Creates and submits monthly reports, including a summary report of persons served.
• Develops and maintains a referral network of regional social service agencies.
• Enters case notes and all necessary information into Client Track database in an accurate and timely manner.
• Attends and contributes to supervisory, staff, and advisory meetings as requested
• Works cooperatively with partner agencies to provide exceptional service to participants
• Performs other duties as assigned.

Qualifications

• Bachelor of Social Work or Bachelor of Psychology degree. Master of Social Work degree, preferred
• Trauma-informed and able to handle crisis situations in a professional, supportive manner
• Previous experience with persons experiencing homeless and HUD reporting requirements, preferred
• Computer literacy to include MS Word, Excel, and Client Track
• Knowledge of Social Work Code of Ethics
• Possess a valid driver’s license
• Occasional flexibility to work schedule
