About Us: The CDC Addressing Conditions to Improve Population Health (ACTion) initiative is a groundbreaking effort aimed at identifying, understanding, and addressing the social determinants of health that impact population well-being. We are seeking a dynamic and experienced Project Manager to lead and coordinate the execution of various projects within the CDC ACTion Grant for the Multnomah County Health Department Chronic Disease Prevention and Health Promotion Unit. This work involves a collaboration across the Health Department, including the Public Health, Integrated Clinical Services, and Behavioral Health Divisions.

The Project Manager will play a pivotal role in the Chronic Disease Prevention and Health Promotion Unit by leading the implementation of the CDC Addressing Conditions to Improve Population Health (CDC ACTion) grant from inception, implementation to completion. The successful candidate will be responsible for overseeing all aspects of the project, including planning, procurement, execution, and management of people, budget, resources, project workplan and scope.
As the CDC ACTion Grant, Project Manager, you will:
  • Lead the planning, execution, monitoring, and closing of projects related to the ACTion initiative.
  • Lead the CDC ACTion grant project from the initial planning stages to its successful completion, ensuring all project deliverables are met within established timelines and budget.
  • Collaborate with cross-functional teams to develop comprehensive project plans, outlining project scope, objectives, tasks, timelines, and resource requirements.
  • Coordinate and manage project activities, ensuring that tasks are completed in accordance with the project plan and within designated timeframes.
  • Procure and allocate necessary resources, including human resources and budgetary allocations, to support project implementation.
  • Manage project risks and issues, identifying potential obstacles and developing mitigation strategies to ensure project success.
  • Collaborate with cross-functional teams to define project scope, objectives, and deliverables.
  • Develop detailed project plans, including schedules, budgets, resource allocation, and risk management strategies.
  • Monitor project progress, track key milestones, and ensure timely completion of deliverables.
  • Manage project budgets, track expenses, and report on financial performance to stakeholders.
  • Foster effective communication and collaboration among team members, stakeholders, and partners.
  • Identify potential risks and issues, and develop mitigation plans to address them.
  • Ensure adherence to project management best practices and CDC guidelines.
  • Provide regular status updates, reports, and presentations to senior management and stakeholders.
  • Lead project-related meetings, facilitate discussions, and drive decision-making processes.
  • Maintain effective communication with internal and external stakeholders, providing regular updates on project progress, milestones, and outcomes.
  • Monitor project expenses and budget allocations, ensuring adherence to financial guidelines and identifying areas for cost optimization.
  • Facilitate collaboration between team members and external partners, fostering a cohesive and productive working environment.
  • Participate and as needed, lead, and evaluate project outcomes and impacts, collecting and analyzing relevant data to assess the effectiveness of interventions in addressing social determinants of health (SDOH) and reducing chronic disease-related disparities.
  • Uphold a commitment to diversity, equity, and inclusion in all project activities, considering the unique needs and perspectives of diverse racial and ethnic communities.
  • Stay informed about the latest developments in project management, chronic disease prevention, nutrition strategies, social determinants of health, health promotion, and Population Health improvement strategies.

TO QUALIFY:

Minimum Qualifications/Transferable Skills*:
  • Education/Training: Equivalent to a bachelor's degree AND
  • Experience: Three (3) to six (6) years of experience that demonstrates the ability to perform the duties of the position.

Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.   • Bachelor's degree in project management, public health, or a related field (Master's degree preferred)
  • Project Management Professional certification or equivalent project management certification
  • Demonstrated commitment to promoting health equity, addressing health disparities, and social determinants of health
  • Proven experience overseeing complex projects overseeing complex projects from initiation to completion in a public health, healthcare, or related setting
  • Strong understanding of chronic disease prevention, social determinants of health (SDOH), and health equity principles
  • Ability to work effectively in a collaborative and interdisciplinary team environment
  • Proficiency in project management software and tools.
  • Excellent organizational, communication, and leadership skills
  • Strong understanding of social determinants of health and their impact on Population Health outcomes
  • Excellent organizational, time management, and multitasking skills
  • Exceptional communication, interpersonal, and leadership abilities
  • Proficiency in project management software and tools
  • Ability to work effectively in a fast-paced and dynamic environment
  • Strong problem-solving skills and the ability to adapt to changing circumstances
  • Experience working with government agencies or healthcare organizations is advantageous

Salary/Pay: Please be advised that the pay range listed for this position is intended to provide general guidance on the earning potential for the role. However, actual compensation will be determined in accordance with the Oregon Equal Pay Law and will take into account factors such as the candidate's relevant experience and education. Hybrid Telework: This position is designated as "hybrid telework," meaning you will be working both remotely (from home) and in person at an onsite location. The designation of Hybrid Telework may be subject to change at a future time. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations. The onsite work location for this position is Walnut Park Complex, 5329 NE MLK Jr Blvd. Portland , OR 97211-3237.
