Job description

JOB SUMMARY:
Manages the Community Health Program and ensures successful implementation of activities in accordance with OCO's service delivery system, local, state and federal regulations, funder requirements and Agency policies and procedures.
JOB DUTIES AND RESPONSIBILITIES:
  • Completes assessments as part of OCO service delivery system, identifies client's needs, and provides referral and follow-up to appropriate OCO and community-based services as needed.
  • Completes referrals to needed services and ensures closed feedback loops, provides community education and outreach to promote health and wellness, manages community consortiums and project teams.
  • Participates in the planning and implementation of Community Health initiatives.
  • Performs activities related to assigned projects, gathers and collects data and information on participant needs,
  • referrals made, and services provided. Leads collaboration with community partners in the realm of substance use, treatment, prevention, and recovery.
  • Provides direct supervision to Community Health data specialist.
  • Advocates and supports client in addressing needs to obtain individual health and wellness goals.
  • Utilizes the Unite Us referral platform receiving referrals, following up on referrals, completing IC&I, and connecting clients to the appropriate OCO line of service.
  • Acts as Project Manager for Health Resource Service Administration RCORP grants. Ensures progress against deliverables in approved workplans and engages in monthly technical assistance meetings.
  • Engages with funder oversight agencies as appropriate and directed. Manages grant reporting and submission activity in state and federal state management systems.
  • Manages budget and ensures funds are expended in accordance with workplan goals for the Health Resource Service Administration RCORP grants.
  • Facilitates the Oswego County Opioid Response Consortium and the Oswego County Healthy Miles Coalition.
  • Manages virtual and electronic platforms for OCO to collaborate internally and with community partners.
  • Responds to consumers who are contacting in person and on the phone to provide information about services and the status of their service. Maintains referral and communication feedback loop.
  • Engages with consumers and provides advocacy, linkage, navigation & retention into services.
  • Works collaboratively with agency staff to complete required reporting associated with project activities.
  • Facilitates, attends, and participates in meetings, gathers and/or prepares and disseminates information, and provides service updates.
  • Provides direct supervision to the Community Health data specialist to ensure accurate and successful reporting for Health Resource Service Administration funding. Organizes and/or facilitates committee meetings, focus groups, and team planning meetings for relevant projects.
  • Prepares/organizes publicity and informational materials; participates in PR activities including community presentations.
  • Organizes and utilizes volunteers and interns for health-related projects; handles the on-boarding process and ensures recognition.
  • Develops and maintains effective working relationships with community partners.
  • Maintains program social media accounts and OCO website for specified program areas. Responsible for OCO database entry across funding streams.
  • Supports programs/departments in the successful completion of program goals.
  • Ensures the collection, maintenance, and reporting of data on consumers served, services provided, and referrals made.
  • Evaluates program progress and ensures activities are completed timely.
  • Makes informed decisions that result in successful program outcomes. Plans and executes community education and outreach events to promote health and wellness in Oswego County.
  • Performs other related duties and special projects as assigned.
  • Ensures that business goals, deadlines and performance standards are met.
  • Facilitates collaborative input and engagement with stakeholders to program goal and deliverables.

JOB REQUIREMENTS:
• Must be knowledge of company policies.
• Must be able to exercise advanced leadership and team management skills.
• Must be proficient in the use of computers, Microsoft software platform and other information technology.
• Must be able to exercise good judgement, solve problems, and gain cooperation from others.
• Must display courtesy, tact and professionalism and be able to work well with the public and Agency staff.
• Must have excellent oral and written communication skills.
• Must be proficient in writing grants, strategic plans, and work plans.
• Must be flexible with schedule and assignments to meet needs of the program(s).
• Must be able to receive direction and follow through independently devising own methods for completion.
• Must be able to provide direction to staff, manage workflows, delegate tasks, provide feedback, assess work
performance.
• Must be detail oriented and thorough and be able to handle multiple priorities efficiently.
• Must have a valid NYS Driver's license with driving record within OCO policy and have regular access to reliable
transportation for travel as needed throughout the County.
• Must have excellent time management skills to ensure tasks are completed across several funding streams.
MINIMUM QUALIFICATIONS:
• Bachelor's Degree in a related field; and
• 1-3 years related experience in managing, planning, evaluating, and reporting or
• Any equivalent combination of education, experience or training.
Full-time - 37.5 hours per week with flexibility required
Grade 11
Monday - Friday 8:00 am to 4:00 pm
