We currently seek a full-time Program Coordinator to join our Children, Youth and Family Services (CYFS). The Program Coordinator will oversee the daily activities and goals of the Women With A Plan program and also the Family Ties Program. In addition, the Program Coordinator will provide supportive services to women and their families impacted by the criminal justice system and utilize individual action plans to ensure goals for each individual are being met.

This position follows a hybrid schedule which is subject to change with or without notice.

Salary Range: $64,000 - $65,600

Essential Duties:

  • Coordinate and supervise intakes for women impacted by the criminal justice system, including family needs assessments for daily living
  • Within the framework of the program, the role encompasses the coordination, assistance, recruitment, and screening of participants, formulating personalized service plans, and facilitating internal and external referrals for services tailored to individual needs, including mental health counseling, public assistance benefits, housing, substance abuse treatment, vocational counseling, family counseling, and any other specific requirements voiced by participants to ensure that every participant receives comprehensive support, empowering them to attain their objectives successfully
  • Formulate and synchronize individualized action plans with participants to promptly address their immediate concerns, ranging from mental health follow-up and housing benefits to family matters and future aspirations.
  • Assign tasks and collaborate with staff and others, including volunteers, to execute designated activities effectively
  • Provide supervision to staff and offer guidance and feedback to elevate the skills and capabilities of the staff.
  • Identify client training interests conducive to career paths and ensure the accurate alignment of referrals with suitable training partners
  • Offer and guide participants to employment and educational resources, addressing their specific requests through connections to both internal and external community resources
  • Facilitate participants' transition into the workforce by supporting their applications for skill training programs, focusing on ensuring their success in building a rewarding career
  • Coordinate and facilitate workshops, as needed, covering topics such as work readiness, professionalism, goal setting, and cognitive behavior
  • Deliver comprehensive case management services, including intensive follow-up, to families and women recently released and identified as high-risk within the scope of Osborne programs
  • Proactively offer support and information regarding the candidate's engagement in the program, meticulously documenting their progress
  • Attends and actively engages in relevant interdepartmental and cross-functional meetings
  • Promptly prepares and submits all necessary reports, participant progress notes, and data utilizing the Electronic Case Management (ECM) system
  • Engage in outreach initiatives to actively recruit and educate the community about the available services as required
  • Ensures operational readiness for audit procedures as necessary
  • Attend required training
  • In person attendance is required to collaborate with co-workers
  • Perform other duties as assigned

Minimum Qualifications:

  • Bachelor’s Degree in Criminal Justice, Human Services, Public Administration, Social Work or a related field; In lieu of a degree, a High School Diploma or equivalent with an additional two years of progressive program management experience is a social service agency
  • A minimum of three years progressively responsible program management experience in a social service agency
  • Knowledge of the criminal justice system
  • Experience providing services to court-involved young adults
