The Role: Part Time PRN Physical Therapist
Join our dynamic team as a Home Health PRN Physical Therapist. You will play a pivotal role in developing and implementing personalized physical therapy programs to improve the lives of our patients. From assessments to effective communication with interdisciplinary teams, your impact will extend beyond the ordinary.
Essential Functions:

• Perform assessments and evaluate patient needs.
• Provide physical therapy services in the comfort of patients' residences.
• Collaborate with patients, caregivers, and interdisciplinary teams in developing and implementing care plans.
• Educate and guide patients and caregivers in physical therapy programs.
• Maintain patient documentation following agency policies.

Qualifications:

• Graduate of an approved school of Physical Therapy.
• Current Missouri Physical Therapy license.
• Current CPR card, valid drivers license, automobile insurance.
