JOB DESCRIPTION SUMMARY
The Registered Nurse Casual Pool (PRN) is responsible for providing services to hospice patients based on initial, comprehensive, and ongoing assessments of the patient's needs and ensuring interdisciplinary collaboration in the development of a patient specific plan of care based on identified needs. The RN is responsible for the provision of direct nursing services in accordance with the plan of care.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care
Complete the initial, comprehensive, and ongoing assessment of patient and family/caregiver needs and provide direct or supervised nursing services based on a developed plan of care.
Assess the patients and family/caregiver's physical, psychosocial, bereavement, environmental, safety, and developmental needs.
Develop an individualized plan of care, in collaboration with the hospice interdisciplinary team, patient, and family, based on assessment, identification of needs, and patient and family/caregiver goals and preferences. Incorporate palliative nursing interventions.
Implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the interdisciplinary group.
Provide care to patients and families through the utilization of interventions and evaluation of outcomes of care.
Perform ongoing assessment and revision of the plan of care, with interdisciplinary collaboration, in response to the changing needs of the patient and family/caregiver.
Perform comprehensive assessment of the patient's pain and develop an individualized pain management plan.
Anticipate, prevent, and treat undesirable symptoms.
Provide support, instruction, and education of the patient, family and other caregivers who participate in the care of the patient in relation to needs identified on the plan of care, including, but not limited to, disease process and progression, medications, pain, symptoms associated with disease, oxygen safety, Hospice philosophy, and care of the terminally ill.
Teach the patient and family/caregiver self-care techniques as appropriate. Recognize and utilize opportunities for health and dietary counseling with patients and families/caregivers.
Work closely with other members of the IDG, an in cooperation with family/caregiver to provide holistic physical, psychosocial, emotional, and spiritual services, including bereavement support to the patient and family to achieve the highest quality of care.
Maintains primary control and professional management of each patient and acts as the primary liaison between physician, patient/family and hospice team to be fully aware of all clinical issues.
Communicate with the physician (Attending and Hospice) regarding the patient's needs, response to treatment, and changes in the patient's condition; obtain/receive physician orders as required.
Maintain an accurate and updated medical record, including all interdisciplinary notes, plans of care, comprehensive and on-going assessments, communications with others involved in patient's care, and any other information pertinent to the care of the patient.
Document nursing assessments, identified problems, measurable goals of care, and limitations to provision of care, care interventions, and response to care in the electronic medical record.
Document all patient related visits and phone calls within the EMR and ensure documentation is completed and transferred (synchronized) at the end of each shift.
Complete, maintain, and submit accurate and relevant clinical notes regarding patient's condition and care given. Record pain/symptom management changes/outcomes as appropriate.
Coordinate all patient and family/caregiver services with the hospice interdisciplinary team and other healthcare providers as needed.
Consult and collaborate with the hospice interdisciplinary team and others involved in the patient's care.
Maintain close contact with the patient's family/caregiver to provide information, support, and continuity of care.
Maintain collaborative relationships with long-term care facility personnel to support patient care and ensure quality symptom management.
Develop the hospice aide plan of care and provide direct and indirect supervision of the aide at least every 14 days.
Maintain the dignity of the dying patient.
Recognize and support the patient's and family/caregiver spiritual and cultural beliefs.
Provide holistic, family-centered care across treatment settings to ensure continuity of care and facilitate attainment of goals of care.
Provide medication review and reconciliation of medication list within the EMR. Ensure medications necessary to ensure patient comfort are available.
Administers medications and treatments as prescribed by the physician in the physician's plan of care as needed.
Maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process.
Provides and maintains a safe environment for the patient.
Works a minimum of three times each month to keep skills and knowledge current. PRN RN will complete and submit a schedule of availability. The schedule is to be submitted to the Hospice Clinical Manager or designee no later than Wednesday preceding the next workweek. Schedule of availability should be in 2-week increments.
Participate in the provision of 24/7 on-call nursing services and assist with coverage of weekend and/or holiday visits to meet the agency and patient needs.
Ensure that arrangements for equipment and other necessary items and services
are available.
Supervise ancillary personnel and delegate responsibilities when required.
Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes and completion of required annual training.
Actively participates in quality assessment performance improvement teams and activities.
Report abuse and neglect in accordance with state laws and regulations as well as organization policy and procedure.
Perform other duties as assigned by the Director and/or Manager.
POSITION QUALIFICATIONS
Graduate of NLN accredited school of nursing and current license to practice professional nursing as a Registered Nurse in the state; Bachelor's Degree preferred.
Current American Heart Association CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Minimum of one years' clinical experience required; hospice experience preferred.
Demonstrate ability to work independently as well as effectively as part of an interdisciplinary team.
Demonstrate autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.
Shows ability to organize and prioritize workload independently.
