As we focus on continually improving outcomes for patients, our network of healthcare providers includes a 25-bed critical access hospital, one of the largest rehab providers in western Colorado, outpatient surgical services, specialty clinics, emergency services, skilled nursing, and assisted living facilities.

ESSENTIAL FUNCTIONS:
  • Determine if the post-acute admission is appropriate given a patient's clinical presentation and advise if other options, such as home with services, is an optimal alternative. Review documentation and/or assess a patient's clinical presentation and collaborate with the provider for optimal alternatives when advised.
  • Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
  • Communicate necessary information to others as appropriate.
  • Monitor patients while at post-acute facilities, including those receiving home health care, looking for:
    • Any deviation from the standard of care for patients
    • Patients being in the post-acute care setting and/or receiving services longer than expected
    • Patients not receiving expected services
    • Patients getting complications in the post-acute setting that puts them at risk for admission to ED and/or hospital
  • Address issues discovered while monitoring patients.
  • Present all findings to a PAC committee, including steps to address the findings.
  • Contact providers, staff and/or administration to resolve the findings.
  • Coordinate discharge planning needs with post-acute facilities to ensure the patient has:
    • Proper follow-up with the PCP and specialists
    • Equipment for transition to home
    • Home health services arranged, starting when the patient is discharged
    • Care management support, including transitions of care
  • Coordinate efforts with facilities' care management programs.
  • Employ effective use of knowledge, critical thinking, and skills to:
    • Advocate quality care and enhanced quality of life
    • Prevent patient complications during post-acute stay
    • Advocate decreased post-acute stay when appropriate
    • Maintain accurate records of all patient-related interactions in hospital's EHR
    • Work in an intensive, fast-paced environment with minimal supervision
    • Ability to stay organized and interact well with others in any situation
  • Excellent computer skills, including proficiency in MS Word and Excel.
  • Proficiency in typing, grammar, data entry, office machines, basic math, and general office procedure.
  • Proficient with standard office equipment (i.e., multi-line phone, fax, copier, etc.).
  • Excellent verbal and written communication skills, including ability to effectively communicate with both internal and external personnel.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Other job duties as assigned.

EDUCATION:
Education: Associate's degree in medical field from an accredited school. Educational background in nursing, social work, or other health related profession.
Licenses: Prefer State of Colorado Nursing License or Licensed Social Worker.
Certifications: No professional certification required.
Experience: 2-4 years working in a post-acute setting, management of eligibility for Medicare Part A coverage for skilled nursing stay and discharge planning.
Knowledge, Skills, and Abilities:
  • Computer proficiency (MS Office - Word, Excel, Outlook) including being able to effectively maintain written and computer records in accordance with regulatory agencies.
  • Establish an approachable demeanor at all times.
  • Strong follow-through, quick decision making, and abilities to effectively problem solve.
  • Team player, ability to build strong relationships and strong teams.
  • Strong verbal and written communication skills with patients, team members, supervisors, and executive level members.
  • Strong organizational and time management skills.
  • Self-motivated, self-starter, ability to set priorities and keep to project schedules.

WAGE RANGE: $29.95-$45.52
Immunizations required foremployment, including COVID-19.
