POSITION SUMMARY: This position is responsible for promoting mental health recovery through a person-centered approach focusing on independence and integration. This is accomplished through delivery of appropriate mental health assessment, transfers, and appropriate termination of care for clients impacted by emotional/behavioral impairment and serious, persistent mental illness who are targeted for mental health services by the Division of Mental Health. This position acts as a liaison for all providers at Four County and external providers as it relates to coordination of care within all service lines. This position is responsible to consistently triage to ensure the appropriate level of care is provided. Work is very broad in scope and requires a high level of professionalism. Works with minimal supervision on most daily activities but must solicit professional assistance in area of responsibility from outside sources, vas necessary. The Primary Care Coordinator reports directly to the Director of respective department/program. The Primary Care Coordinator must exercise tact and judgment and confidentiality in contact with public, clients and families, as well as fellow associates of the Center. The Coordinator must demonstrate the ability and willingness to work in a team-oriented atmosphere, and to provide a clinically relevant and therapeutic environment for clients. The Coordinator must communicate readily with all treatment team members, including internal and external entities. The Coordinator is to ensure that services are delivered in responsibility according to the best professional and ethical standards. The work is broad in scope and requires the application of complex and technical practices and procedure.

- Four-day (32 hour) work week
- Eligible for $150/month ($1,800 annually) in student loan repayment, available at hire
- Earn $750 for every person you refer who is hired and remains employed for 6 months and receive an additional $500 every year as long as you both remain employed

Requirements:
Education:
- At minimum, a High School graduate or GED. A degree or further education in healthcare or healthcare management preferred but not required.
- Must complete and pass all training relevant for designation as Other Behavioral Health Professional (OBHP) and maintain ongoing training/certification provided by Center
Background Checks:
- 4C Health is a Drug-Free Workplace employer. Candidates must be willing to submit to a pre-employment drug screen.
- Candidates must be willing to submit to comprehensive background checks, including but not limited to criminal and child protective services checks.
Experience:
- Experience in care coordination preferred.
- Prefer two (2) years of full-time experience in mental health, or other related field.
Licensure / Certification:
- Possess a valid driver’s license.
- Maintain mandatory agency in-service attendance.
- Obtain and maintain CANS and ANSA Super User Certification
Necessary skills:
- Ability to work under the supervision and direction of a mental health professional.
- Ability to coordinate treatment among all internal and external treatment providers.
- Strong organizational and leadership skills.
Other:
- Knowledge of community resources.
- Demonstrated computer skills preferred
- Will receive training and expected to maintain competency in documentation.
