Position Summary: Primary care reimbursement is undergoing major transformation as payment models align toward value-based care arrangements that reward higher quality, more efficient care delivery. At the core of this transformation is the ability to work with data to understand who our patients are, identify who needs special intervention, and target clinical programs that will drive better health outcomes for our patients.
The PCCS will work collaboratively on a team of clinical care professionals and data analytics experts to accurate research and document clinically appropriate medical coding, with a focus on chronic disease patients. The PCCS will then work with the practices of the IPA to communicate findings and integrate them with the care management process in each office.
Responsibilities and Duties
Ensure the high-quality, timely coding of diagnoses and procedures for IPA practice patients, using ICD-10, CPT and HCPCS coding classification systems.
Research the patient’s longitudinal medical record, including information from the EMR, health plan portals, and HEALTHeOUTCOMES to ensure comprehensive documentation of all relevant diagnoses and conditions.
Prepare summaries and assign the appropriate codes that apply for physician review.
Make corrections to draft reports sent for physician review and submit approved reports back to the IPA practice.
Work closely with the IPA Care Management team, IPA member physicians and staff, and the IPA data analytics team to ensure accurate depiction of a patient’s health.
Work closely with health plans to understand and effectively incorporate health plan policy into coding analysis.
Integrate findings and workflows with the Vatica process in each practice as appropriate.
Understand the quality programs of the IPA, and coordinate coding efforts to integrate with those programs.
Perform other duties as assigned.
Qualifications and Requirements
2+ years’ work experience as a Medical Coder.
Certified Coding Specialist (CCS, CPC, CRC, CCS-P) through AAPC or Registered Health Info Tech/Admin (RHIT or RHIA) through AHIMA
Proficient computer skills.
Excellent communication skills, both verbal and written.
Strong people skills.
