A Prevention Navigator contributes to AHF & CALOR by providing effective and efficient facilitation of prevention navigation services for racial and ethnic minorities in Chicago. The Prevention Navigator is responsible for the provision of outreach, recruitment and linkage to care activities, including accompaniment, follow up services and peer support.  The Prevention Navigator will also be responsible for providing HIV, STI, and HCV testing or referrals to these services. The Prevention Navigator works with other program team members to enhance the number of clients receiving services from AHF/CALOR. The Prevention Navigator will also assist the team in developing and conducting social marketing messages and campaigns and providing ongoing support to encourage retention in services that focuses on substance use. 

 

A Prevention Navigator enhances the reputation of AHF & CALOR by helping CALOR/AHF with its mission to provide compassionate and informed care to our clients.  These duties allow the opportunity to engage within the community and educate those at risk.  Caring, personable and relatable team members enhance the probability of clients returning for services in the future.

To be successful as a Prevention Navigator it is crucial to ensure that the AHF core Values and Mission stay top of mind with all that your do. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. 

 

The Prevention Navigator will work as a part of a close knit team to provide or refer clients to services such as HIV/STI Testing, insurance navigation, HIV care, PrEP, and substance misuse programs . Prevention Navigator will work with other team members and partners to ensure client barriers are addressed.
