General Responsibilities

  1. Operates as a member of the South Area Community Services Team providing screening, assessment, case planning and case management to households who are homeless or at risk of homelessness.
  2. Works with community members including landlords and prospective partners of Catholic Charities.
  3. Capacity to maintain flexible work hours.
  4. Other duties as assigned within the guidelines of this position.

ESSENTIAL FUNCTIONS:

  1. Provides screening, assessment, case planning, and case management and follow-up services to homeless individuals and families in the Homeless Prevention Program and Rapid Rehousing program
  2. Prepares and maintains accurate confidential case records, unit inspection reports, progress notes, fiscal documents and other supporting documentation.
  3. Monitors participant progress and assist them with individualized goals and /objectives towards housing stability and self-sufficiency skill maintenance. Documents services provided ensuring clarity and completeness of information.
  4. Assists in special projects such as Holiday Outreach (Kathy’s Gifts program), Back to School Fair, and Mobile Food Pantries.
  5. Assists with Program Quality Initiatives and some Administrative support functions, including reviewing files, filing, answering phone calls, covering for absence of receptionist.
  6. Advocates, networks and engages with current and prospective landlords to create opportunities for permanent housing for clients, locates rental units in the community, and supports Rapid Rehousing activities for clients including Unit Inspections in rental units through the service area.
  7. Has an understanding of rental leases, housing unit maintenance.
  8. Assists with donations management, organization and client basic supply needs
  9. Provides intake and referral information to prospective clients, callers and facilitates Coordinated Entry processes.

Requirements:
  1. Bachelor's of Arts degree preferred in the human services field (preferred) or a degree in a different field with courses in areas of Human Services, Social Work, Psychology, Sociology, Family and Consumer Sciences. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  2. A minimum of 1 year of experience working with persons from diverse social backgrounds is required.
  3. Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
  4. Strong verbal and written communications skills.
  5. Passage of complete background clearance, and any required testing.
  6. A valid driver’s license, reliable means of transportation and proof of liability insurance is required
  7. Demonstrated capacity to work with individuals and families in crisis situations.
  8. Demonstrated understanding of Human Development
