Job DescriptionOverviewUnder the supervision of the HIV Prevention Care Coordination Manager, the Biomedical Prevention Navigator helps clients enroll in HIV biomedical prevention services, including pre-exposure prophylaxis (“PrEP”) and post-exposure prophylaxis (“PEP”) at Men’s Health Foundation’s clinic in West Hollywood The Prevention Care Coordination team at Men’s Health Foundation works with patients who are at risk of contracting HIV to facilitate access to biomedical prevention services including assisting with health insurance and providing case management services.
Essential Functions and Responsibilities (This list may not include all the duties assigned.)
- Provide individual counseling/assessments for potential PrEP/PEP clients.
- Work with clients to assess eligibility for and enroll clients in supportive programs, including PrEP-AP, patient assistance programs, and to refer for enrollment in MediCal, Covered California-based private plans, and other health insurance coverage for which the client is eligible.
- Assist patients with access to supportive services, including mental health treatment, substance abuse treatment, housing assistance, and nutritional support.
- Document services in All Scripts, MHF’s electronic medical record, county and state databases, as contractually required.
- Collaborate with team members to identify client needs and generate innovative ideas to engage new patients into PrEP/PEP and other medical services.
- Conduct PrEP-related presentations and training to staff, patients, and community members in a variety of settings.
- Attending professional conferences and summits to showcase the work of MHF and to gather the latest biomedical prevention trends and data.
- Identify and participate in outreach events to enlist new PrEP/PEP and/or medical patients.
- Represent MHF in community planning groups, advisory boards and committees.
- Along with biomedical prevention staff, co-produce outreach and recruitment events to bring visibility to PrEP/PEP program.
- Participate in staff meetings and department meetings.
- Other duties, as required.
- A bachelor’s degree in public health, social work, psychology, counseling, nursing, human services, or health education OR A minimum of three years conducting outreach to vulnerable populations with the aim of improving health outcomes.
- Experience and specific training/experience working with people living with HIV/AIDS preferred.
- Bilingual/Spanish preferred
- Ability to work quickly and accurately to elicit information from patients and enter the information into an electronic database without the patient feeling ignored.
- Excellent communication and organizational skills; strong written and interpersonal skills
- Proficient in Microsoft Office (Word, Excel, Access, PowerPoint)

- Demonstrated ability to provide services to people of diverse races, ethnicities, sexual orientations, gender identities, nationalities, economic backgrounds, religions, ages, English-speaking abilities, immigration status, housing status, and physical abilities in a multicultural environment.
- Must be able to pass a pre-employment drug test, physical, and a background check to include a 7-year criminal, 10-year SSN & employer history reference check.
- Must be able to provide proof of COVID-19 vaccination on the first day of work.

- Must be able to work flexible schedules.
- Must take yearly flu shots or wear flu mask during flu season for patient-facing positions and test for tuberculosis as required by the Centers for Disease Control and Prevention.
Must be able to read, write and speak the English language fluently. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Must have the ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public in person.
Mathematical Skills
Ability to calculate figures and amounts such as dollar totals, monetary conversions, discounts, interest, fractions, decimals, percentages, area, and volume.
Reasoning Ability
Ability to solve technical and practical problems and deal with various intangibles in situations where only limited communication, support, information, and access exist. Ability to interpret various instructions furnished in written, oral, diagram, or schedule form.
Computer Skills:
Must have good computer skills and be proficient in the use of systems to complete correspondence, documents, spreadsheets and presentations, such as Microsoft Outlook and Office, Word, Excel and PowerPoint, and Adobe Acrobat.
