Position Summary
The primary responsibility of the Practice Transformation Account Manager (AM) is to manage the relationship and performance of partner medical practices and primary care providers to whom we provide services. The AM will be responsible for developing relationships, onboarding, monitoring and managing quality, utilization and financial performance of assigned medical practices, as well as work in conjunction with the Program Manager to identify new groups or introduce new services.
We are looking for a highly motivated, personable, and collaborative self-starter who is comfortable with a startup environment. You'll need drive, agility, and the ability to drive change while maintaining strong relationships with stakeholders in a cross-matrixed environment.

- Develop and maintain strong relationships with assigned affiliated provider groups, including decision makers and office staff to understand the groups' business operations from processes to workflow.
- Identify key gaps in provider groups' workflow and work with the group to implement best practices or facilitate training sessions in order to achieve greater outcomes for the practice.
- Create reports to display Key Performance Indicators (KPI) for the group. Present and share those reports with all stakeholders monthly and provide recommendations for best practices
- Provide feedback on our delivery of population health services and internal processes to enhance the service offering.
- Develop and lead training sessions for assigned groups for new implementation and ongoing improvement training.
- Participate in new group sales presentations acting as a subject matter
- Continual expansion of knowledgebase to understand the product and enhancements to communicate effectively with your assigned groups.
- Liaison between your assigned groups and the payer related to services provided or other issues identified that could impact relationship
- Serve as the main point of contact for assigned groups.
- Manage time and resources effectively by creating plans and executing in a timely manner.
- Coordinate all activities of data gathering and information, distribution of information and ongoing administration of assigned groups.
- Willingness to travel up to 75% of the time to visit practices in-person within your geographical location
- Responsible for other job-related duties and projects as assigned
This position has no supervisory responsibilities.
QualificationsRequired Education and Experience
- Bachelor's degree or additional related equivalent work experience
- Minimum of 3-5 years of Practice Management or Managed Care Experience
- Experience delivering training
- Use of Microsoft Power point and Excel & G-Suite
- Able to build rapport with ease, have strong business acuity, have a problem-solving mentality with a bias for action and results
- Must be comfortable being able to present, answer questions, and run meetings in both small and larger settings
- Ability to identify and address complex business issues in person.
- Success in managing high touch clients.
- Ability to work in a fast-paced environment while managing multiple priorities and meeting deadlines.
- Excellent oral and written communication skills.
