The role you'll contribute:
The Post-Acute Care (PAC) Liaison will function as a patient advocate as it relates to coordination of post-Acute Care services with the members of the AHS Preferred Provider Network (PPN). This person will assist in identifying patients with post-Acute Care needs, will collaborate with multiple disciplines to coordinate post-acute services, and maintain the relationships with post-acute providers. This person will manage the daily activity of the PPN, including data collection and analysis, and work closely with the Transitional Care Coordinator (TCC) on readmission management and quality monitoring.

Acts as a liaison between the hospital and the post-acute agencies affiliated with the local PPN
Responsible for identifying patients with post-Acute Care needs and collaborating with Care Management, Care Navigation, and the TCC to coordinate these services.
Educate patients/families on the AHS Preferred Provider Network and advocate on behalf of the patient.
Strictly adheres to mandated federal, state, local regulatory and statutory requirements as well as agency or facility compliance policies and procedures for referral processing.
Follow up with PPN-affiliated patients and the respective PAC provider when patients are re-hospitalized during their PAC episode to for quality assurance.
Collects, analyses, and presents PPN data to hospital and PPN agencies on a regular basis.
Promote the growth of AHS post-acute positive public relations and excellent customer service skills.
Maintain a high level of competency, growth and development.
Performs other duties as assigned by management.
EDUCATION AND EXPERIENCE REQUIRED:
Graduate of a Vocational School of Practical Nursing.
Minimum of three (3) to five (5) years experience as a Licensed Practical Nurse.
KNOWLEDGE AND EXPERIENCE STRONGLY PREFERRED:
Knowledge of post-acute (e.g. home health and/or skilled nursing facility) regulations and CMS eligibility requirements.
Mastery of grammatically-correct verbal and written communication
Exceptional customer service skills
Proficient in daily use of Microsoft Office applications, including Outlook, Word, PowerPoint and Excel
High level of multitasking and organization
Able to work autonomously
Project management experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Current registration with Florida State Board of Nursing as a Licensed Professional Nurse
