Job Summary and Qualifications

The Placement Specialist is responsible for facilitating admissions of HCAT patients from HealthONE emergency departments to behavioral health hospital services via management of phone triage. He/she will work closely with all healthcare providers to facilitate and coordinate efficient, effective, quality healthcare. The HCAT Placement specialist must be available for 12-hour shifts with rotating weekends. 

What youll do: 

• Providing services to patients who have complex psychosocial and discharge planning needs.
• Coordinates and facilitates the development of a Discharge Plan of Care for at-risk patient population with discharge needs. 
• Functions as a member of the Interdisciplinary Patient Care Team, who communicates and receives referrals from other patient care providers to help meet the needs of the patient and family for a safe discharge. 

What qualifications you will need:

• High School Diploma/GED required. Bachelor's degree in Social Work, Psychology or related field preferred.
• Minimum of 1 year of experience in mental health, crisis work, experience preferred.
• BCLS through the American Heart Association or American Red Cross required at time of hire and CPI also required within 30 days of hire.
