As the Licensed Practical Nurse, you will partner with the Director of Nursing and Interdisciplinary Teams to further that mission. These passionate teams will depend on you to care for and support our patients during their hospice journey.
The responsibilities of Hospice LPNs/LVNs encompass a range of tasks, such as administering medication, managing symptoms, dressing wounds, providing personal care and assisting patients with activities of daily living. They work closely with other healthcare professionals, such as physicians, registered nurses, social workers and chaplains, to ensure a comprehensive and holistic approach to patient care. Emotional support and facilitating open communication between patients, families and the healthcare team are integral to their role.
Qualifications: - An enthusiasm for providing the best possible care to patients and their families
- An LVN/LPN license in good standing
- 1 year of clinical experience, hospice or home health preferred
- A valid driver's license
- Current CPR certification
- Excellent observation, verbal, and written communication skills
- And additional related duties as needed
