Physician Internal Medicine, Director of Care Management

We are seeking a Physician to serve as our Director of Care Management! You will assess all patients and customizes ideal treatment options and recommend other services offered by the wellness team.

Responsibilities:

• Provide comprehensive care to our patients

• Effectively diagnose patients and suggest a successful course of treatment

• Consult with patients to understand their symptoms and health concerns

• Prescribing medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury

• Explain procedures and discuss test results or prescribed treatments with patients

• Monitor patients' conditions and progress, and re-evaluating treatments, as necessary

• Provide health and wellness advice to patients, including diet, physical activity, hygiene, and disease prevention

• Maintain detailed notes of appointments with patients, including comments, tests, and/or treatments prescribed, and test results.

• Refer patients to other medical specialists, when necessary.

• Provide support and advice to patients receiving care.

• Preparing official health documents or records, when necessary.

• Serve as primary supervising Physician of the Nurse Practitioner(s)

• Sign the Collaborative Practice Agreement for the Nurse Practitioner(s)

​Qualifications:

• Degree from an accredited Medical School. Documented completion of Residency training from an accredited institution.

• Possession and active status of all licensing, board certification, and other requirements as set forth by NC, federal, professional, hospital, accrediting and health payer entities on an ongoing basis. This will include but is not limited to state medical license, payer credentialing and BLS/CPR

• Current DEA license required

• Active malpractice insurance policy

• Strong leadership qualities

• Strong organizational skills
• Excellent written and verbal communications skills
