Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The Physical Therapist is assigned to Home Based Primary Care, a department within Geriatrics and Extended Care at the Washington, DC Veterans Affairs Medical Center in Washington DC, a Complexity Level IB facility. The therapist is assigned to work primarily in HBPC However, the therapist is trained as needed to cross-cover to other areas.
** The Therapist will travel to patients homes to provide care in the home.**

Major Duties:
Provision of Clinical Care:

- Follows safety guidelines and standard operating procedures that.
impact all Physical Therapy patients and programs.
- Responsible for independently providing assessment and treatment interventions for patients received through healthcare provider referrals and patient self-referrals.
- Performs evaluations and examinations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique personal and environmental factors.
- Interprets findings from examination, diagnostic studies/medical tests and, during evaluation, select appropriate clinical tests and measures, and then integrate findings into the physical therapy plan of care for the full range of patient populations.
Establishes a diagnosis within the scope of physical therapy and identify the appropriate intervention to treat and/or refer to another provider for further consultation when needed.
- Determines physical therapy prognosis by incorporating examination findings with the patient's preferences for meaningful level of function in order to set clinically appropriate treatment goals and optimize outcomes.
Documentation
- Performs competent written documentation of initial examinations, progress notes, and discharge summaries in compliance with medical center and APTA standards in a timely manner.
- Demonstrates advanced competency in knowledge of electronic documentation and computer skills.
- Enters consults to Prosthetics service for rehabilitation equipment in accordance with Medical Center policies.
Education
- Acts as Clinical Instructor for affiliating schools for physical therapy for at least 1 student per year.
- Educates other facility personnel in the scope of HBPC services and programs involving a variety of health care specialties.
- The PT continually seeks to improve knowledge and skill by review of current literature, participation in in-service training, and attendance at continuing education programs especially in their area of expertise.
- Continuing education and training will be focused on maintaining and improving competency in all general clinical functions but will also include coursework directed toward the development and improvement of clinical instruction, data collections and reporting techniques, and utilization and reporting of outcome measures.
Administrative Responsibilities
- Maintains state licensure and CPR certification in a timely manner.
- Completes encounter and consult information in accordance with DC VA Medical Center guidelines.
- Uses the computer system for storing and retrieving electronic documents and files, transmitting and receiving e-mail, activating printers, requesting leave, and accessing the Electronic Health Record (EHR).
Providing Leadership
- Assists Supervisor in planning and developing various treatment programs to meet patient needs, as requested by HBPC Program Director
- Actively supports and promotes all programs of HBPC.
- Assists with orientation of new Physical Therapy staff.
Triage and Workload Distribution
- Establishes and maintains contact with other health care providers involved in patient care via written, telephone, and personal communication on a regular basis across the DC VA Medical Center system.
- Resolves problems that interfere with delivery of Physical Therapy service.
Quality Assurance (QA) / Quality Improvement (QI)
- Assists in the development, planning and implementation of monitors of quality of care with respect to HBPC as part of the comprehensive health care services delivered to the patients within the DC VA Medical Center catchment area.
- Conducts record keeping related to patient care documentation and quality improvement (QI) data collection as specified by the Supervisor.
- Reviews and makes recommendations on policies, procedures and supply and equipment acquisition.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: spoken and written English language proficiency in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j. The qualified individual will meet and maintain the qualification requirements for the position, assignment, and grade as outlined in the Physical Therapist Qualification Standard, VA Handbook 5005/99, Part II, Appendix G12.
  • Education and experience: The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a Physical Therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a Physical Therapist. (3) Doctorate degree in physical therapy.
  • Licensure: Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Non-licensed PTs, who otherwise meet the basic requirements in this standard, may be given a temporary appointment as a graduate PT at the GS-11 grade level under the authority of 38 U.S.C. 7405 (a)(1)(D) for a period not-to-exceed two years from the date of employment on the condition that such PT provide care only under the supervision of a PT who is licensed. Failure to obtain licensure during that period is justification for termination of the temporary appointment.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grandfathering Provision. All licensed PTs employed in VHA on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: (1) Employees grandfathered into the GS-0633 occupational series as PTs may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation. (2) PTs who are appointed on a temporary basis prior to the effective date of this qualification standard may not have their temporary appointment extended or be reappointed on a temporary or permanent basis until they fully meet the basic requirements of this standard. (3) PTs initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, meet all the basic requirements of this qualification standard and must maintain the required credentials as a condition of employment in the occupation. (4) If PTs who are grandfathered under this provision leave the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.
GS-11 Experience: Assignment. PTs at this level are responsible for providing assessment and treatment intervention. PTs plan and modify treatment based on a patient's response to intervention and/or change in medical condition. PTs at this level may be given general assignments in any physical therapy program area where advanced specialized knowledge is not required or may serve as the sole PT such as in an outpatient clinic. Licensed PTs may provide oversight and delegate patient care responsibilities to a Physical Therapist Assistant, and/or delegate non-patient care duties to non-licensed staff. PTs that are not licensed must practice under the close supervision of a licensed PT
GS-12 Experience: In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-11 grade level. In addition, the candidate must demonstrate all the following KSAs.
Demonstrated Knowledge, Skills, and Abilities: 1. Ability to coordinate workflow and clinical activities as well as monitor data reports within physical therapy section to ensure optimal patient care. 2. Ability to articulate and communicate to the clinical team the assignment, project, problem to be solved, actionable events, and objectives, as well as to advise on work methods, practices and procedures. 3. Ability to provide new staff orientation, staff development, and training. 4. Demonstrates advanced skill to serve as mentor and coach of the clinical team. 5. Ability to manage staffing requirements and priorities, and coordinate work in order to complete duties in an accurate and timely manner. This includes the ability to follow-up on pending issues and demonstrating an understanding of the impact of incomplete work.
Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety, and be performed by the incumbent at least 25% of the time. The Lead PT reports directly to a Supervisory PT. He/she monitors and makes work assignments, provides input on performance, resolves daily workplace issues and maintains efficient workflow. PTs at this level have experience that demonstrates the possession of advanced practice skills and judgment across many areas of physical therapy for both inpatient and outpatient clinics. Lead PTs are responsible for providing clinical practice guidance for other staff PTs and assist with problem solving. He/she assists PTs in determining treatment options for complex patients with multiple conditions. Lead PTs are responsible for ensuring work is completed by distributing and balancing workload among employees in accordance with established work flow or job specialization; assuring timely accomplishment of the assigned workload; and assessing the status, quality and progress of work, and making day-to-day adjustments in accordance with established priorities. Lead PTs obtain assistance from supervisors or managers on problems that arise. Lead PTs are generally found in higher complexity facilities, or in facilities of lesser complexity levels where large physical therapy departments are found. Individuals may perform ancillary assignments, including program management duties on an occasional basis, where the complexity and amount of work is not substantial.
