About Us: Dacona Medical Center is a multi-specialty clinical practice that provides comprehensive evaluation and treatment services for both pediatric and adult populations. Located in the culturally diverse and medically underserved community of West Tampa. Our facility is equipped with state-of-the-art resources, including a sensory gym, an ADL Kitchen, a sensory calming room, individual treatment rooms, and a spacious physical therapy room complete with exercise equipment, and parallel bars, all designed to support growth, learning, and rehabilitation.

Position Type: PRN (Flexible: Weekday Mornings, Afternoons, or Evenings, and/or Saturday)
Sign-on Bonus: Competitive sign-on bonus offered.
Pay: $50-$60, based on level of experience
Duties Include:
- Conduct thorough patient examinations and evaluations on initial visits upon referral.
- Assess the physical, communication, interaction, and cognitive skills of both pediatric and adult patients.
- Develop individualized treatment plans tailored to the unique needs of each patient.
- Clearly define treatment goals and expected outcomes for patients.
- Continuously monitor and record patient progress and modify treatment plans as necessary.
- Reassess patients at least every 30 days and collaborate with physicians to adjust the plan of care as needed.
- Maintain comprehensive and timely documentation for each patient encounter in accordance with industry standards and regulatory guidelines.
- Provide patient education and advice, and coordinate support for patients, family members, caregivers, and clients.
- Communicate effectively with physicians, family members, caregivers, and other healthcare professionals via phone calls.
- Supervise Physical Therapy Assistants (PTAs) in compliance with municipal, state, and federal regulations.
Education and Experience Requirements:
- Doctor of Physical Therapy (DPT) or equivalent degree.
- Must be a Licensed Physical Therapist in the state of Florida.
- Experience working with both pediatric and adult populations is preferred.
- Strong interpersonal and communication skills.
- Ability to work effectively within a multidisciplinary healthcare team.
