Come join a general practice/orthopedic clinic with opportunity to treat multiple patient types! The clinic is adjacent to Orthotics and Prosthetics and Physician clinic so wonderful access for provider relationships. Our team includes 7 other PT's and an OT/CHT. We are seeking a Physical Therapist with experience with vestibular and concussion treatment, but willing to train to these patient types as well. Acute care support is needed at times (orientation will be provided).
- 0.75 FTE, authorized for 60 hours per pay period.
- Holiday rotation of coverage for acute care with 13 other PT's.

Responsibilities
The Physical Therapist partners with the patient and their care providers while performing evaluation and re-evaluation. Captures and applies pertinent data, performs examination, and resolves treatment diagnosis
Develops an individualized plan of care through coordinationAnalyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals
Implements and modifies care plan based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partners care team to provide skilled interventionImplements and alters treatment to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
Documentation/BillingUses acceptable medical terminology and follows professional practice standards.
Completes documentation in a timely manner per department standards
Stays in sync with department billing standards
Professional ExpectationsPrioritizes tasks and patient care for effective time management.
Understands and follows professional licensure/certification requirements

Qualifications
Required
Physical Therapy registration/licensure in the state of Minnesota or temporary license in the state of Minnesota
Current Basic Life Support (BLS) certification
Preferred
3+ year of PT experience treating vestibular diagnosis
Doctorate degree in Physical Therapy
