We are interested in experienced candidates or eager new grads with excellent clinical and communication skills. Experience and interest working with the geriatric population is a plus but not required. Our team of Physical Therapists is composed of experts in movement, committed to enhancing the quality of life through a combination of prescribed exercises, hands-on care, and comprehensive patient education. We strive to provide outstanding quality care to patients in the comfort of their homes. We are an in-home physical therapy practice under Medicare Part B. We are NOT a home health agency!

Location: Varies

- The Bronx (City Island, Country Club, Locust Point, Pelham Gardens, Pelham Bay Park, Morris Park, Throgs Neck, Riverdale)

Responsibilities and Duties:

• Communicate with the care coordinator to take on and schedule new referrals
• Assess patients for functional limitations within their home
• Perform thorough evaluations and develop patient-centered treatment plans with specific goals in collaboration with the patient, family, and physician
• Assist patients with the use of durable medical equipment
• Perform thorough balance and fall risk assessments
• Document and record all evaluations, prognosis, treatments, progress, and other pertinent patient interaction in a timely manner using our user-friendly EMR
• Educate patients, family, and/or caregivers about in-home exercise programs, transfer techniques, home safety, etc.

Skills and Qualifications:

• DPT degree from an accredited Physical Therapy program
• Valid Physical Therapy License in New York State
• Current CPR certification
• Current Liability/Malpractice Insurance
• Working knowledge of personal computer and software applications
• Outstanding interpersonal and communication skills
• Consistent ability to demonstrate clinical excellence
• Strong desire to develop professionally
• Problem-solving skills
• Time management skill
• Ability to be encouraging, empathetic, and firm

Pay: From $65.00 per hour
