Working hours: 8:00 AM to 4:30 PM
Visiting location: within 30 miles from the above location (miles reimbursement will be given)
Visits per day: 7- 8 
Document Required: Updated Resume, BLS (AHA), PT Certificate/ License 

Licensure Requirements

• Current Physical Therapy licensure in state of practice
• Current CPR certification
