What we’re looking for:

A full-time licensed Physical Therapist able to provide therapy in patients' home, in Brooklyn. If full time is not an option, we also have fee for service opportunities available.

Must have…

• A valid driver’s license

• A degree from and accredited Physical Therapy or Physical Therapy Assistant Program

• Ability to travel around allocated boroughs

• Ability to function with minimal supervision

• Good organizational and communications skills

• Excellent interpersonal Skills

• One year of Home Health experience (preferred but not required)

Responsibilities:

Evaluate patients referred for home care services to determine appropriateness for physical therapy services.

• Based on initial assessment, establishes a Home Health treatment plan with goals in place, and provides treatment in a timely/efficient manner.

• Assures that the agency is ready to meet the patient’s needs by collaborating with agency personnel.

• Explains agency’s policies and services to the patient and responsible family members during initial home care assessment.

• Completes documentation accurately, and submits to agency office in a timely manner.

• Establishes and promotes ongoing communication with Agency personnel.

• Assures customer satisfaction.

Compensation:

85,000-100,000 per year
