Job Description
- Adheres to CMS guidelines by providing initial and ongoing assessments to determine the level of functioning, including OASIS assessment at appropriate time points.
- Gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
- Develop and revise the plan of care in consultation with the physician and other care team members.
- Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
- Preparing clinical progress notes and evaluating outcomes.
- Supervising therapy assistants and home health aides as appropriate.
- Educate patients and/or families about proper safe use and recommend methods for the installation of adaptive equipment.
PT Qualifications and Skills:
- Currently licensed by the Massachusetts Board of Physical Therapy Examiners.
- Experience in acute care settings or Home Health is preferred but not required.
- Able to maintain strict confidentiality about protected and sensitive information.
- Must be able to solve problems independently and confidently by applying analytical and logical thinking.
- Able to read, analyze, and interpret reports, general business directives, policy and procedure statements, and governmental regulations.
- Able to write reports and correspondence at a professional level.
- Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.
