We’re Looking for a Physical Therapist Who:

• Will provide the best patient care around
• Loves working independently, but also working with a collaborative team 
• Is a great communicators, both written and orally.
• Wants to work
  • in home health in the Fond du Lac, and surrounding areas, and/or
  • in Skilled Nursing Facilities in Fond du Lac and Waupun 
• Enjoys working for a company that provides a support system like no other!

Requirements: 

• Graduate of an accredited Physical Therapy curriculum.
• Current license/certification in Wisconsin.
• Current American Heart Association CPR Certification.

 Job Duties include:

• Evaluate and develop individualized patient treatment plans
  
• Work with clinical staff, patient and families to maximize patient’s use of functional abilities
• Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
• Instruct Physical Therapist Assistants in treatment procedures to be utilized and provide supervision
• Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan
