The Home Health Physical Therapist Assistant will work under the supervision of a licensed Physical Therapist to provide physical therapy services to patients in their homes in accordance with the Plan of Care. The assistant will help patients regain their strength and mobility, manage pain, and prevent further injury or disability. The assistant will also work with the Physical Therapist to implement treatment plans, document patient progress, and communicate with other healthcare professionals as needed.
- Assist the Physical Therapist in evaluating patients and following treatment plans
- Provide physical therapy services to patients in their homes
- Help patients regain their strength and mobility, manage pain, and prevent further injury or disability
- Document patient progress and communicate with other healthcare professionals as needed
- Provide education to patients and their families about their condition and treatment plan
- Adhere to all state and federal regulations and guidelines for physical therapy services
- Associate's degree in Physical Therapy Assistant program
- Current state license as a Physical Therapist Assistant
- Minimum of 1 year of experience in home health or related setting
- CPR Certification
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
