Job Summary
FYZICAL Therapy & Balance Centers | Granger is a medical based fitness and wellness facility offering Physical Therapy Services, Group Fitness, Personal Training, Massage, and so much more. Our current facility has been established in the Michiana Area for over 20 years. We are now relocating to a new, State-of-the-Art facility to facilitate health, fitness, and recovery in our community. At FYZICAL, we create connections, delivering more than treatments. Job Responsibilities

• Actively demonstrates FYZICAL's core values and brand promise and conducts oneself in a manner that is consistent with these values.
• Comply with regulatory agency standards and regulations and knows/adheres to all laws and regulations pertaining to patient health, safety, and medical information.
• Evaluate new patients.
• Create and implement goal-oriented treatment plans. Establish goals that are functional, measurable, and attainable.
• Treat patients and conduct re-evaluations to alter the plans of care and update the goals as individually required.
• Assists patient towards goal achievement through effective treatment plan and interventions. Develops home exercises and self-treatment plans for the patient.
• Provides education to patient and family, including diagnosis and prognosis of therapy.
• Responsible for the accurate and thorough completion of, evaluation reports, progress notes/flow sheets discharge summary and any additional required documentation.
• Performs other duties consistent with purpose of job as directed.
• Implementing and complying with FYZICAL's PTRx model through all client communications and actions. Meeting and exceeding clinical performance metrics.
• Complete knowledge of FYZICAL's proprietary services and offerings. Includes but not limited to: Vestibular Rehabilitation Therapy, Orthopedic Services, Pelvic Health Services, Fitness Services, Wellness Services, and Retail Products.
• Report to the Clinical Manager as directed and to discuss and correct any potential patient care or documentation issues.

Job Requirements
Education and Licensing

• Successful completion of a DPT Program required.
• An active Physical Therapist license in the state of Indiana is required.
• Must earn FYZICAL's Balance I, Body-Q, Class IV Laser, and Orthotic Screening Certifications within 30 days of employment.

Required Skills

• Vestibular Rehabilitation and Balance Therapy experience preferred.
• Training in Pelvic Health and Wellness Preferred.
• EMR or "Electronic Medical Records" knowledge is preferred.
• Professional appearance and the ability to eloquently interact with patients and their family members.
• Following all CDC and Health Department Guidelines to maintain a safe and sanitary workspace. Assist with maintaining the clinic's equipment and facility standards.

Compensation and Benefits:

• $35 - $45 per hour, dependent on experience and certifications.
