Job Duties:

• Actively demonstrates balance's core values and behaviors and conducts oneself in a manner that is consistent with these values.
• Comply with regulatory agency standards and regulations and knows/adheres to all laws and regulations pertaining to patient health, safety and medical information.
• Evaluate new patients.
• Create and implement goal-oriented treatment plans. Establish goals that are functional, measurable and attainable.
• Treat patients and conduct re-evaluations to alter the plans of care and update the goals as individually required.
• Assists patient towards goal achievement through effective treatment plan and interventions. Develops home exercises and self treatment plans for the patient.
• Provides education to patient and family, including diagnosis and prognosis of therapy.
• Responsible for the accurate and thorough completion of, evaluation reports, progress notes/flow sheets discharge summary and any additional required documentation.
• Performs other duties consistent with purpose of job as directed.
• Report to the Clinical Manager as directed and to discuss and correct any potential patient care or documentation issues.

Job Requirements

• Education and Licencing
• Successful completion of a Bachelor's Degree required.
• An active Physical Therapist license in the state of Indiana is required.

Required Skills

• Pelvic Health and Wellness experience preferred.
• Vestibular Rehabilitation and Balance Therapy experience preferred.
• EMR or "Electronic Medical Records" knowledge is preferred.
• Professional appearance and the ability to eloquently interact with patients and their family members.
