Locations:
•White River Junction, VT/Lebanon, NH area
• Physical Therapists at the VNH screen, evaluate, re-evaluate,and treat patients as well as coordinate and communicate treatment activitieswith our interdisciplinary team members and the patient.
The PT will:
Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
•SupervisePhysical Therapy assistant and contribute to the professional development ofnew staff and students.
ProductivityRequirement:
•5 pointsper day. (SOC 2, routine visit 1, eval 1.5, OASIS DC 1.5, recert 1.5, ROC 1.5)
Minimum Qualifications:
•BS inPhysical Therapy
•Must be alicensed Physical Therapist in VT and NH, or hold a compact license.
One year of experience as a PT in a home healthcare setting
